Summary Title: HBCI Program Manager Eligible for Flex Schedule: Yes Location: Community Based/Hybrid Essential Personnel: Yes Hours: 37.5/1950 HBCI Interventionists work with families when they are in crisis and begin safety planning with the family within 48 hours of the referral. Each family will collaborate in creating and receive a written copy of the safety plan outlining triggers, warning signs, supports and steps to ensure the safety of the physical environment. Caseload consists of up to 3 families for 4-6 weeks for crisis services. Services are provided in the family home and community at times convenient for the family. Interventionists are available 24hours/day 7 days/week including weekends and evenings. The Interventionist is a critical member of the HBCI (Home Based Crisis Intervention) team. The Interventionist is responsible for providing treatment to youths aged 5-20 years and their family/caregivers to address the clinical needs of the child and the complex needs of the family unit. The Interventionist will provide crisis intervention and stabilization with the child/youth and family, and case management services such as referrals to other services and ensuring the structural home environment is safe. Interventionists use individualized treatment interventions and evidence-based practices to address identified clinical and family system needs. The Interventionist will carry a caseload of up to 3 recipients. The HISS will complete Intakes, in person, at Elmira Police Station and will provide staffing oversight and support services to clients at the shelter/warming station/motels. Contact at both locations will primarily be with clients who are experiencing homelessness and/or housing crisis. The HISS, will likely encounter clients that have experiences with long term homelessness, mental health, and substance abuse concerns along with other barriers that impact their ability to maintaining suitable/safe housing. In both locations, the HISS will enter client information into the Homeless Management Information System through AWARDS. Essential duties for this position are empathy, effective communication skills, problem solving skills, crisis de-escalation, multi-tasking, attention to detail, solution-based results, and ability to be mobile. ELMIRA POLICE STATION: the HISS is responsible for in person completion of; intakes, screening assessments and applications (Temporary Assistance Application, Health Screening, ILP, 4002, CE), after hours motel reservations and scheduled transportation to motel/shelter/warming station. SHELTER/WARMING STATION: The HISS will receive clients being referred from the ELMIRA POLICE STATION Essential Duties and Responsibilities • Provides coverage at the Elmira Police Station location and the shelter/warming station as scheduled., • Intake Services:, • Has creative conversations about housing solutions to support clients.., • Maintains an understanding of agency housing programs and regulations, to provide clients a solution to their circumstances to secure the optimum housing solution possible., • Solves challenges, problems, and barriers wherever possible., • Inquires and documents client income and resources., • Completes assessments and applications with client(s)., • Enters client information and progress notes into HIMIS/AWARDS software system., • Manages the Master List for the Department of Social Services outside of regular business hours., • Reviews and completes Homeless Housing expectations and guidelines., • Maintains regular dialogue with the Emergency Services Case Manager, through documentation and conversation, ensuring the needs of the clients are addressed., • When not on site at the Elmira Police Department, hotline calls require mandatory responsiveness to report to Elmira Police Department, not to exceed 15 minutes., • Supports and completes general office and clerical duties., • Attends team meetings as scheduled., • Shelter, Warming Station, Motel Support Services:, • Provides coverage as scheduled or needed., • Regularly monitors all areas of facility regarding cleanliness, storage, appearance, and safety of all clients/staff., • Completes maintenance work orders for facility noted during shift., • Completes incident reports as applicable per the agency procedures., • Develops an approach that will work with each client to create an environment that is calm and cohesive., • Responsible for all functions required for day-to-day operations of the facility, during shift, i.e. supervision of meal preparation, supply inventory, transportation, medication oversight, chores, upcoming appointments, all in collaboration with the Emergency Services Case Manager., • Ensures needs of guests at motels are met as needed during shift.
