Job Description Case Manager The Case Manager is responsible for case management services for clients in transitional housing. The Case Manager will have an assigned caseload of 25 cases. He/She will be responsible for successfully integrating each client's housing and employment goals and ensuring compliance with Win's contractual requirements with outside Funders. Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. The Nassau County PINS (Person In Need of Supervision) Diversion program is designed to divert youth who are at risk, from PINS petitions that require court involvement. PINS Diversion services families with children up to the age of 18 in Nassau County. Assist clients in moving into permanent housing by providing support such as attending lease signings, assisting with applications for one-shot deals, providing moving assistance, etc., • Provides additional information and instruction on the skills required to secure and maintain permanent housing, such as how to create and maintain a household budget., • Participate in housing meetings, case conferences, staff meetings, and other meetings/trainings as assigned., • Documents all counseling, escorting, referral and discharge planning activities in client charts., • Provide comprehensive case management services to an assigned caseload., • Provide referrals for in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate. REQUIRED QUALIFICATIONS: • High school diploma, G.E.D or high school equivalency is required, • 2 years experience related to housing application/placement functions, • Experience providing services to individuals with mental illness, substance use, chronic health conditions and/or homelessness., • Sensitivity to issues involved in working with a diverse organization., • Must have initiative, the ability to work well under pressure, and the ability to work both independently and as part of a team, • Strong communication and organizational skills. Job Description Essential Job Functions: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients, • Attempt to reach by phone and mail victims who have filed reports with the precinct or PSA with a specific focus on victims of violent felony crimes and those victims who have filed reports two or more reports in the previous 12 months, • Work with victims of crime who come to the precinct or PSA seeking assistance, • Conduct home visits with officers and detectives to assist victims as needed, • Provide information, referrals, and practical assistance to victims experiencing crime, • Conduct client centered safety assessments and safety planning suited to the individual’s needs, • Provide crisis intervention and supportive listening, • Advocate with NYPD personnel that may include keeping a victim apprised of an NYPD investigation, • Provide criminal justice advocacy, • Advocate for victims for public benefits they may be entitled to, • Help with applying for compensation through the New York State Office of Victims Services (OVS), • Coordinate community resources for crime victims, • Provide information and referral for victims for services such as shelter, mental health treatment, medical care, and other entitlements, • Provide emergency practical assistance for victims including transportation and food vouchers, • Work collaboratively with police officers to effectively aid victims, • Collect data, track the history of crimes, and prepare monthly reports, • Perform data entry into a computerized record keeping system and maintain thorough accurate, and timely case records and statistics, • Advocate with Family and Criminal Court staff for victims, • Provide coverage in vacant precincts as needed, • Conduct trainings with police personnel, • Conduct community presentations, • Attend relevant trainings, workshops and seminars, • Work with other victims of domestic violence as needed, • All internal Safe Horizon programs and departments that can comprehensively support the client and case plan, • All external agencies and organizations that can provide appropriate services to the client, specifically the NYPD and the court systems, • The district attorney’s office in each of the boroughs, including the domestic violence bureaus, • Clear and concise verbal and written communication skills, • Excellent interpersonal skills, be assertive and well organized, • A minimum of two years’ experience in crime victim service delivery, or other relevant experience, • Basic knowledge of crime victim and domestic violence issues, • Basic understanding of computer and systems for managing case records, • Bilingual ability may be required to meet individual precinct/community linguistic needed This position is on-site.
