Job Description Overview: We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our skilled nursing facility rehab unit and long-term care unit This position offers the opportunity to provide comprehensive medical care, manage chronic conditions, and collaborate closely with interdisciplinary teams to ensure high-quality patient outcomes in a supportive and dynamic environment. Locations: • Schofield Residence Nursing Facility - 3333 Elmwood Ave, Buffalo, NY 14217, • Fox Run at Orchard Park - One Fox Run Ln, Orchard Park, NY 14127, • Premier Genesee - 278 Bank St, Batavia, NY 14020, • Livingston County Center for Nursing & Rehabilitation - 11 Murray Hill Dr, Mt Morris, NY 14510, • Fred & Harriett Taylor Health Center - 7571 NY-54, Bath, NY 14810, • Absolut Care-Three Rivers - 101 Creekside Dr, Painted Post, NY 14870, • Elcor Nursing and Rehabilitation Center - 48 Colonial Dr, Horseheads, NY 14845, • Patient Care:, • Conduct thorough initial assessments, develop care plans, and perform routine follow-up evaluations for patients in the rehab and long-term care units., • Manage acute and chronic medical conditions, including but not limited to diabetes, hypertension, respiratory diseases, and neurological disorders., • Prescribe appropriate medications and treatments, adjusting them as necessary based on patient response and evolving care needs., • Collaboration:, • Work closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans., • Participate actively in interdisciplinary team meetings to discuss patient progress, treatment plans, and discharge planning., • Documentation and Compliance:, • Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements., • Complete documentation in a timely manner, including progress notes, medication orders, and discharge summaries., • Patient Education:, • Educate patients and their families on disease prevention, treatment options, medication adherence, and lifestyle modifications to promote overall well-being., • Provide counseling and support to patients facing complex medical decisions or challenges., • Quality Improvement and Training:, • Participate in quality improvement initiatives, clinical audits, and performance improvement projects to enhance patient care outcomes and facility practices., • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of NY., • Certification as an NP or PA from an accredited program., • Previous experience in geriatrics, rehabilitation, or long-term care preferred., • Strong clinical assessment skills and the ability to make autonomous clinical decisions., • Excellent communication skills and the ability to collaborate effectively within a multidisciplinary team., • Schedule flexibility, • Competitive salary and benefits package., • Opportunities for professional development and continuing education.
