Job Description Summary: The Substance Use & Family Support Specialist will serve as the substance abuse specialist for the Clinical Supports Program. Provides consultation and engagement support to CPS Teams working with families impacted by substance use disorders. Assists with assessment, harm reduction, treatment linkage, and safety planning to support child and family well-being. This position will work directly with CPS staff to provide trauma-informed consultation, crisis intervention, and guidance to ensure families receive timely access to appropriate services. The Substance Use & Family Support Specialist will act as the primary liaison between CPS teams, community providers, and partner agencies to streamline referrals, improve coordination of care, and ensure consistent responses for families. This position requires flexibility, as some cases may require after-hours or weekend response depending on client needs or emergency situations. Responsibilities: · Integrate a trauma-informed, client-centered, and culturally responsive approach into all clinical collaborative activities. · Serve as the designated substance abuse liaison between CPS, community partners, and external providers. · Conduct brief assessments and provide crisis intervention and psychoeducation to families. · Support CPS staff with consultation, case planning, and strategies to address complex family dynamics. · Facilitate warm handoffs and referrals to appropriate community services, ensuring follow-up and coordination of care. · Conduct substance use screenings, assessments, and brief interventions using evidence-based practices. · Train CPS teams on risk reduction strategies, safety planning, and treatment options. · Facilitate referrals and warm handoffs to appropriate treatment providers. · Participate in case conferences, home visits, and discharge planning meetings. · Deliver borough-based training for ACS staff on addiction and recovery. · Collaborate with hospitals, treatment programs, and community providers. · Document activities and outcomes in required systems. · Develop and maintain relationships with hospitals, outpatient clinics, and mental health partners to expand service access. · Participate in multidisciplinary team meetings to provide mental health expertise and ensure care planning. · Deliver mental health awareness and self-care training to CPS staff to enhance trauma-informed practice. · Maintain accurate and timely case documentation in compliance with program standards and confidentiality requirements. · Respond to after-hours or weekend mental health needs as required by emergent situations or program demand. · Participate in team meetings, supervision, and professional development opportunities. · Perform other related duties as assigned. This will include coordinating closely with the Manhattan Felony Alternative to Incarceration (ATI) Court to identify and assess potential participants. The ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director, the Senior Counselor, Jail Diversion will lead a team that conducts independent clinical assessments and drafts treatment recommendations for defendants being held in pretrial detention. The Senior Counselor will supervise a team of counselors and oversee the referral, assessment, and treatment planning process—ensuring adherence to a strict timeline and that the process is completed as quickly as possible. They will also regularly conduct assessments and prepare treatment plans, including for more complex and demanding cases. Additionally, in coordination with the Clinical Director, the Senior Counselor will design and refine workflows for the team, and oversee data entry. The goal is to provide information that will enable defense attorneys, prosecutors, and judges to resolve cases as quickly as possible, significantly reducing the time people spend in pretrial detention. An essential and dynamic feature of the Senior Counselor’s role will be close collaboration with key external stakeholders. They will work regularly with New York City’s public defense organizations and other defense attorneys, prosecutors from the Manhattan District Attorney’s Office and the Office of the Special Narcotics Prosecutor, staff at the Department of Correction and Correctional Health Services, the judge and court staff from New York County Supreme Court who oversee the Felony ATI Court, and an array of community-based alternative to incarceration and community-based providers. Court-based clinical work is an exciting, unique and demanding area of practice, and the Senior Counselor role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. Job Description About the Role The Youth Counselor will be primarily responsible for building healthy relationships with each person supported and providing adequate care, safety, custody, control and counseling with the aim of allowing the youth to become a productive member of society. What You’ll Do • Provides direct supervision and be accountable for whereabouts of persons always supported., • LSP only: Utilize mechanical restraints to transport youth between sectors of the home., • Works closely with representatives from the Department of Education to ensure safety during educational enrichment., • Intervenes during emergencies or altercations to ensure the safety of all youth., • Managed persons supported behavior utilizing the principles of the MYSI Model with the goal of teaching the young people to make positive, lasting changes in their behavior., • Implements daily living routines, schedules and procedures., • Assures the safety of the persons supported and maintains security, • Ensures high standard of sanitary conditions throughout the house including heavy cleaning and laundry., • Participates in the planning and execution of educational, recreational, and personal enrichment activities., • Participates in ongoing meetings, conferences, training programs as required., • Interacts and communicates with Rising Ground staff in matters related to, but not limited to, the needs of the residents and the program, in general., • Attend all mandatory training required by ACS, OCFS and the Agency., • Responsible for adhering to all ACS, OCFS and agency regulations., • Uphold rules, repertoire of responses, confrontation techniques, application of the restriction system, negotiating restrictions, Force Fields, utilization of Safe Crisis Management techniques when necessary., • Manages the peer group, understands peer group dynamics, promotes and maintains a positive peer group culture., • Attention to and support of medical issues, student educational needs, casework, clinical appointments, etc. Monitors persons supported in Family Court., • Completes log reports, complete behavior assessment reports, and submits documentation, incident reports., • Informs the Case Planner and supervisors of any issues related to the youth or their families as they become aware of such information, • Ability to work various hours and days of the week, • Performs other duties as assigned Your Qualifications • High School Diploma + 3 Years of Direct Services in Mental Health or Juvenile Justice Programming, • Associate’s degree + 2 Years of Direct Services in Mental Health or Juvenile Justice Programming, • Bachelor’s Degree + 1 Year of Direct Services in Mental Health or Juvenile Justice Programming, • Valid NYS Driver’s License required., • Must have exceptional verbal and written communication skills in order to maintain required documentation, • Must have a commitment to work from a strength based and/or youth development and trauma-informed perspective, • Commitment to the mission and values of Rising Ground, • Must be able to pass required Safe Crisis Management (SCM) and recertify annually, • Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities required of the employee for this job. Supervisory Responsibility: No Work Environment: Work locations are at an assigned juvenile justice facility throughout the five boroughs. Position Type/Expected Hours of Work: This is a 40-hour nonexempt position. Mandatory overtime is to be expected based on the needs of the program on various shifts. This is not a work from home position. Staff are paid for 8 hours straight time and are expected to eat with residents during mealtimes. Travel (if required): None- Program travel is only during program hours. Physical Requirements: Must have the ability to complete all standard administrative and support tasks including but not limited to climbing up and down stairs, operation of computers/phones/fax/printer/copy machine, and the ability to lift boxes, furniture, and equipment up to 30lbs. Must be able to travel from the various locations and sites of Rising Ground. Ability to perform physical restraint of youth using approved restraint techniques. Physical clearances are required and reevaluated annually.
