Program/Department Description Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. Community Access’ IMT Teams serve individuals who are street homeless or living between institutional settings and housing. Team members work collaboratively with settings in which the participant is living and use assertive engagement strategies to proactively engage individuals in services. IMT team members share responsibility for the people they serve, helping individuals attain housing and other personal and safety goals that may be of a health, social, work or education nature. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant’s choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week Position Overview The IMT Assistant Program Director is responsible for assigned administrative and management tasks and for the provision of direct services. The IMT Assistant Program Director supports the IMT Program Director in the overall daily operation and management of the IMT program, including clinical support to staff and direct program participant care. It is not acceptable for a client or their family to tell a HHA not to disturb a client when arriving or leaving. The crew will promptly notify the dispatcher of the results of the inspection. In the event a violation notice is issued, the crew must contact a supervisor prior to accepting another patient. The supervisor will decide the course of action. Our policies and procedures are geared toward strict compliance with the New York State Department of Health regulations. In the event a violation results from an administrative shortcoming, management will accept responsibility and promptly correct the procedure. In the event a violation results from failure on the part of the attendant, disciplinary action will be taken. End of tour: The crew must return the ambulance to the garage, secure the ambulance and equipment as required, replace all supplies used during the tour and then report to the supervisor. This position provides safer use supplies, personal care items and pantry, health education, and assistance with referrals. Overdose prevention education is conducted on a regular basis to all clients with a history of substance use. Mobile services are conducted utilizing a team approach in collaboration with testing, navigation, mobile medical services, outreach, and behavioral health staff in the department. This position is also responsible for driving the mobile units and assisting with vehicle maintenance. Services are conducted utilizing a harm reduction approach that supports the client in identifying their own goals around substance use and safer sex. We support clients who are actively using, those who use occasionally, and those who are in recovery. The Justice Center’s court responds creatively to low-level offending, seeking sentences that are restorative to the defendant and community. In addition, the Justice Center works upstream and outside the courtroom through diversion initiatives, street outreach, and community engagement. The Midtown Community Justice Center is seeking a Temporary Youth Engagement Specialist to lead the Justice Center’s work with young people from Amsterdam Houses and the surrounding community. Reporting to the Community Partnerships Coordinator, the Youth Engagement Specialist will play a key role in both developing and overseeing youth programming at Amsterdam Houses and supporting the Justice Center’s broader community engagement initiatives across the borough of Manhattan. By working with community stakeholders, local leaders, community-based organizations, and the young people themselves, the Youth Engagement Specialist will seek to understand quality of life and public safety challenges for the community then co-lead the Justice Center’s programming built in response to those findings. This role will, among other things, provide tangible support and coaching to the program members, and will support residents, community partners and local city agency representatives through the process of identifying and solving issues that create vulnerabilities in community safety and well-being. The Youth Engagement Specialist will also be principally tasked with supporting the Justice Center’s Youth Advisory Board.
