Position Overview In this role, working alongside caring, dedicated, and experienced senior nursing leadership and interdisciplinary team, the Assistant Director of MDS and Case Management oversees and directs the Resident Assessment and Reimbursement, Case Management, and with the Rehabilitation and Facility Transitional Care programs. Reporting to the Director of MDS and Case Management, the Assistant Director of MDS and Case Management Services also serves as the Nursing Department liaison to Finance and Rehabilitation Services. Job Description Director of Reflections/Memory Care Coordinator We are seeking a caring, dedicated and experienced LPN nurse supervisor to become the Director of Reflections, our dedicated Alzheimer’s and dementia Memory Care Neighborhood at our beautiful and luxurious community of The Bristal Assisted Living at North Hills. In this crucial role, you will be responsible for the day-to-day management and success of the Reflections Program, including planning, organizing and coordinating special needs programs. You will also provide team member supervision and training, and act as a liaison and advocate for our residents and their families.
