Job Description MHA promotes mental health and total wellness for individuals facing challenges associated with mental illness and addiction recovery. We increase community awareness while enhancing mental well-being through advocacy, education, prevention, treatment, and services. Benefits of working at the Mental Health Association: • Joining the staff of supportive and team-oriented individuals, • MHA is an approved site for the New Jersey Behavioral Healthcare Provider Loan Redemption Program., • Excellent benefits package available, • Retention Incentive provided after completion of 6-month anniversary Qualifications:, • A Bachelor's degree in a human services field, or an Associate's degree and two years’ experience in providing human services, or five years of human service experience, or possess a relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, bachelor's degree in social work, certified rehabilitation counselor, certified alcohol and drug counselor, bachelor's degree in rehabilitation, or bachelor of science in nursing., • Computer literacy required.Responsibilities:, • Responsible for daily implementation of Partial Care Program, in collaboration with staff, supervisors and members, to provide partial care services to individuals with severe and persistent mental illness, to enhance community functioningMilieu Activities:, • Plan and facilitate therapy, coping, activity and psycho-educational groups to enhance social skills and promote relapse prevention and independent living., • Collaborate with members and staff to accomplish psycho social goals and tasks of the unit., • Provide milieu management., • Participate in unit trips including van driving, • Evening, weekend and holiday social program coverage as required by program needs.Case Management:, • Provide case management services for members as assigned, including coordination of medical, psychiatric, financial, residential, social, and legal needs of assigned members., • Regular goal-setting with members., • Crisis intervention, • Assist members to obtain entitlements, especially Medicaid/Medicare, food stamps., • Obtain medical records., • Liaison with family, boarding home staff and local agencies which serve the elderly., • Consult regularly with Prospect House program psychiatrists., • Consumer advocacy and outreach., • Arrange transportation services., • Provide transportation as needed.Record Keeping:, • Maintenance of electronic clinical records including case notes, treatment planning, outcome data and other reports as required by funding sources.Program Development:, • Collaborate with Program Director and Unit Leader to develop program to promote growth and expansion of services, select supplies and materials, research new program methods and activities., • Maintain contact with other programs for information sharing, referrals and public relations., • Input into weekly scheduling of staff.Other:, • Participate in computer training as required., • Participate in outreach/marketing efforts as required., • Must meet all expected productivity levels, in regard to direct service provision to consumers., • Perform other duties as assigned by the Director. Annual Base Rate: $44,000 Reports to: Program Director and Program Coordinator Fully paid medical coverage for eligible staff. Generous 401(k) match.
