In this role, you will be responsible for screening and conducting intakes for individuals seeking admission to the Wyandanch Clinic. Conducts new client intakes and coordinates with all referral sources and collaterals. Maintains appropriate records according to program regulations. Provide clinical evidence-based services with a focus on integrated mental health, physical health, and substance use recovery. Service delivery will occur on-site, in vivo, and in the community. A caseload of adults, children, and families will be maintained while providing individual, group, and family sessions. QUALIFICATIONS: Licensed Clinical Social Worker (LCSW).
