Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities Under the supervision of the Medical Director of Mental Health or a supervising Psychiatrist in the Division of Correctional Health Services, this full-time position requires the provision of high-quality, team-based psychiatric care, including psychotherapeutic and psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. The psychiatric service strives to provide safe, holistic, and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units (mental observation units) for individuals with serious mental illness, a 24-hour outpatient-level clinic in each facility, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. Each of the 12 facilities in the jail system has unique patient population and environmental characteristics; each Psychiatrist will be matched with the population that most closely meets their clinical expertise and desired work location. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines including security staff is critical. • Provides psychiatric care, including admission evaluations, follow-ups and urgent and emergent consultations to individuals enrolled in the mental health service, • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication, • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics, • Regularly participate in the creation of individual treatment plans for patients, and review plans already in place, • Evaluate patients for the need for a higher level of care, including hospitalizations. Job Description Overview Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Physician’s Assistant will be matched with the population that most closely meets their clinical expertise and desired work location and schedule. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization and escalate care when clinically appropriate. Work Shifts 8:00 A.M – 4:00 P.M Duties & Responsibilities Purpose of Position Under general supervision of the Director, Engineering and Support Services, assists in planning, directing, coordinating and evaluating capital programs of the Corporation, including General Construction, Telecommunications, Energy Conservation and other Engineering Support Programs. In collaboration with the Director, formulates and evaluates programs and projects of the office and supervises the functional programming activities of discrete units directed toward accomplishing the division’s objectives. Examples of Typical Tasks • Plans and develops engineering projects concerned with problems which have a major impact on the operation and function of a designated unit., • Directs, coordinates and maintains control over project assignments and related activities., • Prepares reports, develops and prepares engineering management audits, studies, surveys for Director of Engineering and Support Services and/or other Corporate officers detailing proposed new programs, statistical studies, leasing property, health facilities planning with recommendations and other data as required. Monitors progress and implementation of approved recommendations., • Maintains liaison with Federal, State and City agencies pertaining to engineering design, construction and other related areas., • Plans and implements programs to establish performance standards to attain increased productivity and accountability., • Supervises project activities through the assignment of qualified key personnel by establishing and maintaining project budget, equipment, material supplies and necessary services and by obtaining and furnishing personnel with basic job information., • Attends staff meetings, reviews capital and expense budget submissions and provides recommendations regarding proposed projects and programs., • Participates in collecting and providing management information to record and report status of projects, assignment and cost by Capital Budget Control and other departments., • May provide consultation services to Executive Directors pertaining to engineering, construction design projects and other engineering support programs and functions., • Coordinates efforts and cooperates with Departmental Managers and Directors on matters of mutual concern and extends functional guidance and assistance., • Participates in developing and preparing standard methods and procedures for implementation of maintenance operations, construction contracts, capital projects development and support service program activities. Provides technical assistance in review of existing maintenance and engineering procedures, service and construction contracts and preventative maintenance for all Corporate facilities. A Baccalaureate Degree in Engineering or Architecture from an accredited college or university; or,, 2. New York State licensure as a Professional Engineer or Registered Architect; and,, 3. Five years of experience in Construction, Design, Engineering or related fields in a health care environment, including three* or two# years in a supervisory capacity; or,, 4. A satisfactory equivalent combination of education, training and experience. Work Shifts • Time: 8:00 AM - 4:00 PM, • Days: Monday to Friday, • Facility: Trailer 4 at Rikers Island Duties & Responsibilities This position works under the direction of the Assistant Chief of Service, Medicine, exercises wide latitude of independent judgment and initiative in supervision of overall administration and management of the Medicine Department. The position assumes full responsibility and authority in areas delegated. May direct a group of services across multiple jail facilities. Responsibilities include: (Detailed Task) • Serve as the clinical director for Complex Care, providing clinical supervision to clinicians (1 NP, 1 RN) who supporting care delivery and coordination for the more medically complex patients in the jails., • This role encompasses the following responsibilities:, • Establishing clinical criteria for inclusion., • Defining the overarching vision and program objectives., • Attending weekly Complex Care rounds to ensure the effective delivery of care., • Consulting on Complex Care patients and offering clinical guidance and support to front-line providers within jail facilities., • Providing direct patient care at least 50 percent of schedule, • Serving as a clinical point of contact for colleagues from other departments as needed., • Delivering patient education related to complex diagnoses., • Ensuring that patients are assigned to medically appropriate housing areas on Rikers Island., • Providing clinical input and support for medically appropriate discharge plans for patients being discharged to the community, including necessary medications and follow-up appointments., • Ensure safe transitions and coordination of care between the jail and external facilities, including the following tasks:, • Obtaining and reviewing external medical records for comprehensive patient assessment., • Reviewing the daily hospital census of Correctional Health Services (CHS) patients., • Collaborating with the Complex Care Nurse Practitioner to schedule appropriate specialty appointments, track these appointments, and communicate care plans to all relevant parties within CHS to ensure proper execution., • Engaging with hospitalists and inpatient specialty teams when CHS patients are hospitalized and as they transition back to the jail or community., • Arranging expedited admissions to the hospital and ensuring the success of planned admissions for Complex Care patients., • Collaborate with other CHS teams regarding shared patients, with particular emphasis on those involved in Mental Health Discharge Planning and Re-entry and Transition Services., • Participate in clinical release work and support legal advocacy in concert with the Clinical Court Advocacy (CCA) team by:, • Acting as a resource to resolve clinical inquiries from CCA staff (social workers) and providing input regarding patient eligibility for CCA services., • Interfacing with legal representatives to furnish relevant clinical information pertinent to a patient’s legal case., • Composing letters that detail medical conditions for legal documentation., • Assuming on-call responsibilities for critically ill patients, ensuring timely communication with hospital staff, CHS leadership, and legal representatives regarding urgent cases., • Engage in the planning, development, and implementation of programs, policies, procedures, and evaluative monitoring techniques designed to fulfill the objectives established by CHS and Health and Hospitals Corporation (HHC) leadership.
