Qualifications and Competencies • High School Diploma + 5 years human services OR Bachelor’s degree., • Organizational and communication skills for working with staff, clients, families, and community., • Critical thinking and judgment skills required. General Description Provide supportive counseling and assistance training individuals diagnosed with mental illness in independent living skills. Promote greater independence in day-to-day activities, personal care, skill building, and community integration. Duties and Responsibilities • Participates in planning and evaluation of service plans and monitors objectives., • Writes progress notes monthly or as needed., • Trains and assists residents in daily living skills., • Monitors medication., • Provides transportation and accompanies residents to activities., • Offers counseling, crisis management, and behavioral interventions., • Assists in daily management of the residence., • Administers residence in absence of Senior Counselor., • Documents emergencies and notifies supervisors as needed., • Case management, follow-up, and other duties as assigned.
