Job DescriptionDescription: Primary Facility Duties ? Assume primary responsibility in an emergency situation and maintain competency with all emergency operational procedures. ? Apply nursing knowledge to ensure the Resident’s needs are adequately and appropriately assessed and met. ? Oversee and ensure that each resident receives a timely assessment and Plan of Care (POC) at admission and at least once every 6 months thereafter, or sooner if there is a change in condition. ? Actively participate in the review of resident’s care to ensure the appropriateness of billing. ? Collaborate with the primary care physician to ensure compliance with physician orders per regulations. ? Accept and retain only those clients who can be safely and adequately cared for with input from external and internal staff, the Resident’s primary physician, and family members. ? Maintain ongoing infection control surveillance, including compliance with OSHA standards, universal precautions and basic sanitation and safety in the facility. ? Ensure the accuracy of Resident records and documentation in adherence to company policies and Department of Health and Senior Services regulations. ? Coordinate care with other department heads, the Resident’s physician, and other third-party providers. ? Other duties as assigned by Administrator or Chief Clinical Officer. Medication Administration Oversight (Primary Facility) ? Oversee medication room including storage, administration, and destruction of medications. ? Ensure that all self-medicating residents are capable of self-administration as set forth by the regulations of the Department of Health. ? Ensure that all Medication Room staff receive proper training and pass the competencies required for safe medication administration. ? Ensure regular and effective communication with all physicians involved in residents’ care to assist with Medication Reconciliation, especially for residents seeing multiple doctors. HHA Oversight (Primary Facility) ? Ensure all paraprofessional staff (PCA/HHA/Medication Aides) receive in-service education on health/wellness related topics, compliance topics, and other relevant topics as per the regulatory requirement. ? Interview, hire, train, supervise, evaluate, promote, and terminate HHAs with Administrator. ? Refer Resident service problems to the appropriate Department Managers, Case Management Director, and Administrator as necessary. ? Develop and maintain accurate Resident records in compliance with company policy and regulatory rules. ? Supervise and evaluate as HHA personnel are providing care to Residents. ? Conduct periodic health seminars for the Residents and/or Residents’ families in collaboration and coordination with the Activities Director ? Participate in the review, development, and implementation of policies and procedures relating to the Agency/ALP, in conjunction with Administrator. Regional Oversight · Oversee and support site RNs in clinical operations of assigned facilities under the Agency licensure. · Directly manage all RNs and/or LPNs working for assigned Agency including interviewing, hiring, training, performance management, and termination, in conjunction with Administrator. · Provide leadership in promoting and maintaining standards for delivering high-quality care by all members of the resident care team. ? Act as a resource for staff to address resident’s clinical concerns and questions. ? Conduct in-person site visits once at a minimum of every 6 months or more frequently based on findings and need. ? Provide orientation to newly hired RN’s for facilities assigned under the Agency. ? Provide or arrange clinical coverage as needed for facilities with staffing constraints. ? Responsible for supporting and driving quality standards through meeting regulatory requirements and the practice of Quality Assurance (QA) including QA tools as directed by Administrator. A responsibility of this job position has clearance for access to the following confidential information: · Patient Clinical files which include all patient scare information, medical orders, assessments and reassessments, medication reports, progress notes. · Agency staff with access to computer files holds all information in strictest confidence in the processing, storage and discarding of all date. Only authorized personnel will have access to written and computer data information; Authorized personnel will be assigned passwords/access codes to computer files necessary to conduct their responsibilities. Requirements: The Regional Director of Patient Services (DPS) is responsible for the coordination of daily clinical operations within their primary facility along with assigned additional facilities, ensuring the provision of quality resident care, executed through direct performance of the Agency/ALP services and functions, in compliance with applicable regulations. QUALIFICATIONS: Required: ? Current RN license in the areas managed. ? Associate degree in Nursing PLUS two years of experience as a professional nurse in community care or related field; or ? Baccalaureate degree in nursing PLUS one year of experience as a professional nurse in community care or a related field. ? BLS Certification Preferred: ? UAS Certified ? Prior ALP, LHCSA, SNF, or LTC experience ? Experience managing care staff and medication technicians.
