Key Responsibilities • Adhere to care plan to ensure safe and competent care for the patient., • Assist patients with personal hygiene and support the maintenance of a clean, safe, and healthy environment., • Help patients with ambulation as prescribed by the physician and approved or supervised by a clinician or therapist., • Support therapy personnel as needed during rehabilitative processes., • Encourage patients to achieve maximum independence according to the nursing care plan., • Promote mental alertness by engaging patients in activities of interest., • Provide emotional and psychological support to patients and household members, fostering trust and confidentiality., • Report any changes in the patient’s condition or home environment to the RN or clinical manager., • Perform light housekeeping tasks related to creating a safe and comfortable environment, as directed by the clinician., • Complete visit reports promptly and accurately within the electronic medical record system., • Confirm daily visit schedules to ensure seamless coordination with other staff., • Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required., • Understand and adhere to the agency’s process for managing and addressing customer complaints., • Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies., • Must have a CNA or HHA license., • Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services., • Must be free from health problems that may be injurious to patient, self, and coworkers and must present appropriate evidence to substantiate this., • Ability to comprehend the basics of personal care, housekeeping, and meal preparation., • Must understand and respect patient needs including ethics and confidentiality of care., • Must have a valid driver’s license and access to a reliable and insured vehicle., • Ability to visit homes that may not be wheelchair accessible., • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques., • Personalized, One-on-One Care: Help patients heal and regain their independence by delivering individualized care in the comfort of their homes., • Growth & Stability: Over two decades as Florida’s largest home health agency., • Ongoing Professional Development: Free Continuing Education Units (CEUs) to support licensure and career advancement., • Competitive Benefits & Perks: Including Daily Pay (work today, get paid tomorrow!) and an employee referral program where you can earn rewards., • Recognized Excellence: Ranked as a USA Today Top Workplace., • Flexible Scheduling: Enjoy a schedule that aligns with your personal priorities.
