Job Summary We are seeking a Psychiatric Consultant to support our growing Collaborative Care Model (CoCM) program. This role provides psychiatric expertise through case review and consultation in partnership with Behavioral Health Care Managers (BHCMs) and Primary Care Providers (PCPs). This is a non-patient-facing role focused on population-based psychiatric consultation, clinical guidance, and collaborative treatment planning. All services are delivered remotely in compliance with CMS CoCM guidelines. Responsibilities • Participate in scheduled psychiatric case reviews with BHCMs (typically biweekly or as clinically needed), • Review BHCM-prepared case summaries, including:, • Diagnoses and relevant medical/psychiatric history, • Current medications and recent changes, • Behavioral health screening scores (e.g., PHQ‑9, GAD‑7, MOCA, BIMS), • Clinical progress, concerns, or safety considerations, • Provide evidence-based psychiatric recommendations, including:, • Diagnostic clarification, • Medication recommendations, • Behavioral and environmental strategies appropriate for SNF/LTC settings, • Risk and safety considerations, • Collaborate with BHCMs to ensure recommendations are clearly understood and ready for PCP review, • Document recommendations in a HIPAA-compliant registry or EHR, • Electronically sign consult documentation in accordance with CMS requirementsRequirements, • NP with board eligibility or certification in Psychiatry, • Active, unrestricted medical license, • Experience with Collaborative Care, integrated care, or consultation-liaison psychiatry preferred, • Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day, • Comfort working in remote, team-based care modelsRequired Training, • Completion of a minimum 6-hour AIMS Center CEU-certified Collaborative Care training (required prior to or shortly after onboarding)Benefits, • Competitive salary:, • Health, • Dental, • Vision, • 401K, • Paid- Subscription to Up-to-date, • Company-Sponsored Life Insurance, • Paid Time Off, • Malpractice InsuranceWhy Work With Us?, • Make a meaningful impact on the lives of seniors, • Work in a collaborative, mission-driven environment, • Fully remote, flexible schedule, • Opportunity to impact care across skilled nursing and long-term care settings, • Strong operational and clinical support structure The above-noted job description is not intended to describe, in detail, the multitude of tasks that may be assigned but rather to give the applicant a general sense of the responsibilities and expectations of this position.
