Job DescriptionPosition Purpose: The Clinic Supervisor is responsible for assisting in planning, coordinating, and monitoring all operational and financial aspects of the Wesley Health Center. In conjunction with providing effective leadership to clinic staff, the Supervisor oversees the delivery of quality, cost-effective patient care. The position collaborates with the Clinic Administrator in directing support services and personnel to discharge their responsibilities. Job Description Purpose: Hope the Mission provides a safe, low-barrier, and supportive 24-hour residence to persons experiencing homelessness. Clients will receive shelter, food, case management & housing navigation services to support them in obtaining permanent housing. Working under the supervision of the Program Manager, the Assistant Program Manager leads and oversees all programmatic and operational functions for the site, provides oversight to shelter support staff, and fulfills the mission by successfully providing leadership and supervision for the program. The Assistant Program Manager ensures that new projects, supportive services, and housing programs are in-line with best and evidence-based practices for working with people experiencing homelessness (e.g., Trauma-Informed Care Principles, Harm Reduction, and Housing First). This position requires being on-call as needed to respond to emergencies and driving (Travel: 25%). Specific Duties-Programmatic * Provide overall direction and oversight of all programmatic components including accurate, up-to-date, data collection, quality assurance, and contract management adhering to guidelines set for in the Scope of Work. Job Description JOB TITLE: DIRECT SUPPORT PROFESSIONAL 1 Position Description: Support adults with developmental disabilities to accomplish daily living activities at the person’s home and/or in the local community. Help the individual to be a known and valued member of his/her local community. The attendant is directly responsible for personal care and support to meet the goals and objectives of the individual that they serve. The Direct Support Professional 1 (DSP 1) shall be aware that the consumers may terminate individual service. Abide by all home rules, property agreements and equipment warranties., 9. Perform all duties as specified by the consumer, the Direct Support Professional 2, and 3 (DSP 2, and 3) and the Area Coordinator as detailed in the ISP and the Daily Task List. Qualifications and Requirements Education and Experience: High School Diploma or equivalent is preferred. One year of experience providing care to persons with developmental disabilities. Experience working with seniors in a human service field is also acceptable. Transportation: a. Valid California drivers license. b. Driving record for the past 7 years (DMV printout required yearly). c. Current vehicle registration and use of a vehicle in good running condition during shift hours. d. Valid automobile insurance for that vehicle and all passengers, with employee name listed on the policy (new verification required at the expiration of the policy). Other: • Current CPR and First Aid Certification for adults and children., • Cell phone with data plan and app capabilities., • Successfully complete a criminal background check through fingerprint clearance.
