We partner with local agencies, funders, and community stakeholders to deliver programs that promote health equity and strengthen community resilience. JOB SUMMARY The Community Family Resource Coordinator (CFRC) provides community-based outreach, engagement, and case management services to individuals and families in need of mental health, social, and supportive services. This role is responsible for identifying community members in need, conducting intakes and needs assessments, and connecting clients to appropriate services through resources, referrals and follow-up. The CFRC delivers services in home, school, and community-based settings rather than clinical environments and focuses on prevention and early intervention through culturally responsive outreach and engagement strategies. The position requires strong relationship-building skills, the ability to navigate sensitive situations, and adherence to program, funder, and regulatory requirements. KEY RESPONSIBILITIES Client Engagement and Case Management • Identify and engage individuals and families needing support through outreach and referrals, • Conduct comprehensive client intakes and needs assessments in a sensitive and nonjudgmental manner, • Develop rapport and build trust with clients from diverse backgrounds, • Identify appropriate services and provide resources and referrals to community-based programs and providers, • Provide follow-up to ensure services are accessed and client needs are addressed, • Utilize referral platforms (e.g., Unite Us, One Degree, or similar systems) to coordinate services, • Maintain accurate and timely documentation of all client interactions, assessments, and referrals, • Ensure all documentation meets program, funder, and compliance requirements, • Maintain confidential client records in accordance with HIPAA and program standards, • Conduct outreach in community settings including schools, partner agencies, and community-based organizations, • Participate in outreach events, community meetings, and presentations, • Recruit and engage potential clients into services, • Build and maintain relationships with community providers and stakeholders, • Work closely with internal program staff to ensure coordinated service delivery, • Participate in team meetings, case reviews, and reflective supervision, • Collaborate with multidisciplinary teams and community partners, • Support program quality improvement efforts and compliance initiatives, • Knowledge of community resource systems, behavioral health services, and social services, • Ability to build trusting relationships with diverse populations, • Strong communication, interpersonal, and engagement skills, • Ability to assess client needs and provide appropriate referrals, • Strong organizational and documentation skills, • Ability to manage multiple priorities and meet deadlines, • Cultural humility and experience working with diverse populations, • Ability to function as part of a multidisciplinary team, • Proficiency in Microsoft Office and data entry systems, • Ability to maintain confidentiality and adhere to HIPAA requirements, • Local travel within the Antelope Valley region required for outreach, home visits, and community engagement, • Occasional travel to off-site trainings, partner meetings, or agency events may be required, • Services delivered in home, school, and community-based settings, • Mileage reimbursement will be provided in accordance with AVPH policy REQUIRED QUALIFICATIONS • Bachelor’s degree preferred in Social Work, Psychology, Public Health, Human Services, or related field, • OR minimum of two (2) years of experience in community outreach, case management, or related field, • Experience working with underserved or high-risk populations, • Experience conducting client intakes and needs assessments, • Valid California driver’s license, reliable transportation, and current auto insurance required, • Must pass background clearance and fingerprinting requirements, • Bilingual (Spanish or other languages reflective of the community) strongly preferred, • Experience with referral platforms (e.g., Unite Us, One Degree), • Knowledge of Antelope Valley community resources, • Experience working in prevention and early intervention programs (MHSA or similar), • Experience with audits, compliance, or quality assurance processes, • Experience working in home or community-based service delivery models ADDITIONAL INFORMATION • This position is grant-funded and contingent upon continued funding and program needs., • Must maintain regular and punctual attendance in accordance with agency standards., • Must comply with all AVPH policies, procedures, and regulatory requirements., • Professionalism & Confidentiality – Maintains discretion and ethical handling of sensitive information, • Community Engagement – Effectively connects with and serves diverse populations in community settings, • Communication – Communicates clearly with clients, staff, and community partners, • Collaboration – Works effectively across multidisciplinary teams and community systems, • Data Management & Compliance – Ensures accurate documentation and adherence to program requirements, • Work is performed in office, school, and community-based settings, • Regular interaction with diverse families, including individuals experiencing high-risk or vulnerable circumstances, • Must maintain professional boundaries, discretion, and confidentiality in accordance with HIPAA and agency policies, • Ability to sit, stand, walk, bend, stoop, and squat for extended periods of time, • May require lifting and carrying materials or equipment up to 25 pounds, • Local travel within the Antelope Valley region required
