Contractor staff shall review a computer-generated, pre-deployment health assessment form with everyone going through the process. Individuals are asked by the HCP/CP to accurately complete the form. The HCP/CP also completes a review of the individual’s MEDPROS to determine what deficiencies the individual might have, and any such deficiencies are annotated on the form. The individual then proceeds to each station. MEDPROS is again checked at each station. The stations are Laboratory, Immunizations, Dental, Vision Screening, and Audiology. When all stations are complete, the patient is seen by a physician/PA/NP who reviews all the data and discusses any health concerns with the patient. If a health issue is identified that requires further evaluation, the patient is seen in Case Management where a file is opened, and appointments are made with the appropriate clinic. o Demobilization: The process beings with a series of briefings which include administrative, medical, and entitlement briefs. Currently, a post-deployment health assessment form is completed by the patient prior to leaving theater. The completed form is reviewed with the individual by the HCP/CP who notes any changes. MEDPROS is checked and the HCP annotates any changes onto the form. The individual then proceeds through the following stations: Laboratory, Immunizations, Vision Screening, and Audiology. When all stations are complete, the individual is seen by a physician/PA/NP who reviews the form and discusses any health concerns that the individual may have. If health issues are identified, a consult is placed, and the individual is seen by a case manager who opens a file and schedules an appointment with the appropriate clinic. o Other Services: Other services to be conducted by the Contractor may include, but are not limited to, post-deployment health reassessments, periodic health assessments, Welcome Center soldier assessments, and active component readiness assessments. DUTIES: · Perform all procedures commonly performed by medical practitioners in their specific specialty and level of licensing and for which privileges have been granted. These shall be essentially the same duties as those required of any Medical Corps officer or government civil service provider of similar experience and in similar duty assignments. Duties shall consist of, but are not limited to, the following: · Provide medical examination, care, and treatment to patients. Review medical histories and progress of patients. Examine patients, make diagnosis of disease, disaster, or injury, and treat patients or prescribe a course of treatment. Abide by all facility rules, regulations, and policies. · Initiate requests for tests, x-rays, and admissions to the MTF, as appropriate. · Follow-up on abnormal reports. Abnormal x-ray and lab reports received on SMs seen by HCP shall be reviewed by the HCP at the time these reports are available for review. Each report shall be reviewed within one week of its receipt. · Prescribe medications, as appropriate, IAW privileges and regulations. · Seek advice of consultants when appropriate. · Facilitate patient access to medical care by recommending appropriate referrals to other HCPs IAW policies and regulations. · Prepare and document appropriate history physical examinations, and diagnosis. Maintain documentation of all treatment provided IAW applicable directives and prepare such records and reports as may be required. Ensure all documentation and reports are legible and complete. · Utilize paraprofessional staff as appropriate. · Respond to cardiopulmonary arrest as appropriate. · Advise the medical director of the clinic where contracted services are being provided of any problems encountered or anticipated in connection with meeting the needs of patients treated. Continuing problems of any nature shall be reported by the Contractor in writing to the COR. · Attend and participate in patient care reports, patient care conferences, team conferences, professional staff conferences, and other appropriate professional activities only to the extent that such attendance and participation is relative to their assigned cases and/or performance of contract services as determined by the COR or DC. · Always adhere to the Center for Disease Control (CDC) guidelines concerning universal precautions and the Infection Control Program while providing services under this contract. · Perform all duties in such a manner that they meet all JC standards. · Indicate responsibility for the contents and accuracy of all prepared reports/forms by signing all such documents/records. · Complete each outpatient encounter in Armed Forces Health Longitudinal Technology Application (AHLTA) at the end of each day or within 72 hours and appropriately close the record.
