Job Description JOB DEFINITION: The Psychiatric Nurse Practitioner is to provide Psychiatric Assessments and medication management to children and adults experiencing a behavioral health crisis within a 24 hour per day, 7 day per week to Crisis Stabilization Center. EDUCATIONAL REQUIREMENTS: • Masters' degree in advanced nursing practice in psychiatry from an accredited program., • New York State license and registration as a Nurse Practitioner in Psychiatry., • Current DEA registration., • Certification required in Medication Assisted Treatment (MAT) (i.e., suboxone). EXPERIENCE REQUIRED: Substantial clinical experience with the diagnosis and treatment of the entire spectrum of mental disorders, in outpatient and/or inpatient settings. Experience will include management of seriously and persistently ill clients as well as those with psychiatric and co-occurring substance abuse disorders. Familiarity with current DSM criteria. DUTIES AND RESPONSIBILITIES: • Provides psychiatric and substance abuse care and medication to appropriate clients applying standards of care consistent with good medical judgment and current practice in the psychiatric community. This can include the administration of intramuscular medication when appropriate., • Provides psychiatric and substance abuse examination for the purpose of diagnostic evaluation, determination of treatment needs and prognostic factors. This includes using all reasonable methods to detect the presence of danger to self or others and collaboration with the treatment team to implement appropriate precautions., • Monitors the pharmacological effects of prescribed psychotropic medications and notifies the team of their reported effectiveness., • Is available for consultation with staff regarding relevant issues in the client medical background and collaborates with appropriate professionals in the community to integrate agency treatment with overall medical care., • Contributes to the timely production of all necessary reports to other agencies concerning clients who they have examined and/or treated., • Prescribe for appropriate laboratory testing indicated in collaboration with the treating physician and/or conducts any other relevant studies or tests. Implements any follow-up needed., • Efficiently uses time management skills to maximize availability for direct client contact., • Collaborates with other members of the treatment team in providing, • integrated care. This may include appropriate outreach when indicated., • Establishes and maintains appropriate and timely clinical case records, including, but not limited to, treatment plans, progress notes and treatment plan reviews, documenting appropriate therapeutic intervention with each client to the Agency, funding, licensing and billing authorities., • Works as part of the ICSC team; attends and participates in team meetings to provide input/feedback around psychosocial conditions/comorbidities to review client status, update plans and goals, review outcomes to further program goals., • Assist the program to provide crisis intervention and follow-up on an as-needed basis 24 hours a day, 7 days a week. Including shared on call responsibility with other PNP., • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress, • Complete QI Training during onboarding and participate in QI refresher training as needed., • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. Provides educational and training services to staff and clients. ESSENTIAL JOB DUTIES AND RESPONSIBILITIESAdministrative (10%):•Participates in department and interdisciplinary committees which affect policies related to the role of the Advance Practice Clinician and delivery of health care to the customer.•Participates in agency quality risk management program. Direct Clinical Care: (80%)•Responsible for assessing, planning and administering health care which is individualized to the patient and consistent with the policies and protocols of PPFA and PPGNY.•Secures a complete health history and performs physical examinations with special emphasis on the reproductive health care, and other types of more specialized reproductive care including abortion care, and transgender hormone therapy.•Provides medications and contraceptive devices as needed.•Monitors medical aspects of all medical services offered at the health center, acting as a resource to center staff and patients regarding medical concerns.•Performs, orders and interprets appropriate laboratory tests.•Provides education and appropriate management for women and men in need of reproductive related services, including fertility control, infertility, and sexually transmitted infections.•Assesses and manages gynecological conditions and provides treatment consistent with PPGNY protocols, policies, and procedures.•Ensures appropriate medical follow-up on all patient testing and/or treatment, including direct patient contacts and laboratory test review.•Create & maintain medical documentation in an EHR system in compliance with EHR policies and procedures.•Provides postpartum care in selected settings within an interdisciplinary team framework with appropriate pre-service and in-service education.•Seeks consultations as necessary from Medical Director or designee.•Collaborates with physicians and other members of the health care team and makes referrals to medical and community resources as indicated. Training: (10%)•At the discretion and direction of the Clinical Director, AVP of clinical implementation and or Medical Director, assists in training and orientation for newly hired APC’s and new skill development.•At the discretion and direction of the health center management, assists in training and orientation for newly hired clinical support staff.•Attends agency sponsored staff meetings, medical in-services and trainings.
