Job Description TransCare Program Coordinator In Our Own Voices, Inc. (IOOV) Albany, NY | Full-Time, Exempt | On-Site (Evenings & weekends required as needed) Reports To: Director of Interventions About In Our Own Voices, Inc. In Our Own Voices, Inc. (IOOV) is a Black-led, LGBTQ+ people-of-color–centered nonprofit organization based in Albany, NY. IOOV is dedicated to advancing the health, safety, and empowerment of LGBTQ+ communities of color through culturally affirming health services, advocacy, and community-building. Our work is rooted in racial justice, gender equity, and liberation-centered care. The TransCare Program is a cornerstone of IOOV’s LGBTQ+ Health Services, delivering HIV, STI, and HCV prevention, education, testing, and linkage-to-care services designed specifically for Transgender and Gender Non-Conforming/Non-Binary (TGNCNB) communities of color. Position Summary The TransCare Program Coordinator is a leadership role within IOOV’s LGBTQ+ Health Services team. This position is responsible for coordinating, implementing, and expanding culturally responsive HIV/STI/HCV prevention and care initiatives for TGNCNB communities of color. The Coordinator oversees evidence-based and community-rooted interventions, supervises program staff, facilitates outreach and engagement, and ensures high-quality, compliant service delivery. This role blends direct service, program leadership, staff supervision, and community engagement to reduce health disparities, strengthen care access, and build affirming systems of support for TGNCNB individuals. Major Responsibilities Program Coordination & Implementation • Coordinate comprehensive HIV/STI/HCV prevention, education, testing, and linkage-to-care services for TGNCNB communities, with a focus on TGNC people of color, • Implement, recruit for, and facilitate evidence-based and homegrown interventions, including Project Phoenix and SexTransations, • Expand community-based and site-based testing services to increase the number of TGNCNB individuals who know their HIV/STI/HCV status, • Conduct risk assessments and provide direct case management, patient navigation, referrals, and linkage to affirming care, • Design and implement social marketing and health education campaigns focused on sexual health, wellness, and empowerment, • Plan, coordinate, and facilitate community events including Transgender Day of Remembrance, Gender-Affirming Drives, and the Sex Toy Drive, • Conduct in-person, digital, and community-based outreach to ensure visibility, trust-building, and sustained engagement, • Conduct HIV and HCV risk assessments, testing, and counseling services in compliance with state and federal guidelines, • Ensure timely and accurate submission of counseling, testing, and outreach data to the NYS AIDS Institute and IOOV funders, • Maintain strict compliance with HIPAA and NYS HIV Confidentiality Laws, • Supervise and support the Case Manager, Community Engagement Specialist, and Peer Advocate, • Provide onboarding, training, coaching, and performance evaluations for supervised staff, • Submit accurate and timely monthly program reports to IOOV leadership and funders, • Participate in staff meetings, cross-departmental collaboration, trainings, and professional development opportunities, • Perform additional duties as assigned to support program growth, compliance, and organizational priorities Qualifications Required • Bachelor’s degree in Public Health, Social Work, Human Services, or a related field, • Minimum of 2 years of experience in clinical or community-based HIV/STI/HCV interventions, counseling, and psychosocial assessments, • Strong knowledge of social service systems and resources supporting TGNCNB communities and people living with or at risk for HIV/STIs/HCV, • Demonstrated understanding of LGBTQ+ health, PrEP, PEP, gender-affirming care, and harm reduction approaches, • Strong group facilitation, written, and verbal communication skills, • Ability to work flexible hours, including evenings and weekends, • HIV counseling and testing certification, or willingness to obtain within 90 days (training provided), • Spanish language proficiency, • Supervisory or team leadership experience Physical Demands & Work Environment • Ability to lift up to 50 pounds unassisted, • Prolonged sitting and extensive computer use, • Close visual acuity required for detailed computer-based work, • Frequent interaction with clients, colleagues, and community partners via phone, email, and in person, • Primarily office-based with regular community outreach and event-based work Why Join IOOV?
