Job Description JOB DEFINITION: Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. EDUCATIONAL REQUIREMENTS: A Bachelor's degree in social work, psychology, rehabilitation counseling, or related field. Licensure with the NYS Education Department in a specific field is preferred.
