Job Description The Physical Therapist will provide high-quality care to each assigned patient, resident, or client with daily nursing care and services in accordance with the resident/patient/ or client's assessment and plan of care. Worksite: Momentum at South Bay for Rehabilitation and Nursing340 East Main Street, East Islip, NY 11730 Click Here to see Location on Map Responsibilities: • Conduct comprehensive assessments of residents' physical abilities, mobility, and functional limitations., • Develop individualized treatment plans based on assessment findings, resident goals, and physician referrals., • Implement evidence-based physical therapy interventions to enhance residents' strength, balance, coordination, flexibility, and overall mobility., • Provide therapeutic exercises, manual therapy, gait training, and other modalities to improve residents' physical function and independence., • Collaborate closely with interdisciplinary healthcare professionals, including occupational therapists, nurses, and physicians, to ensure a coordinated approach to resident care., • Evaluate the need for assistive devices, orthotics, and mobility aids to enhance residents' safety and mobility., • Educate residents and their families on proper body mechanics, therapeutic techniques, and home exercise programs to optimize treatment outcomes., • Monitor and document residents' progress throughout the course of therapy, adjusting treatment plans as needed., • Adhere to legal, ethical, and professional standards of physical therapy practice., • Participate in team meetings, case conferences, and in-service training to enhance clinical skills and knowledge., • Stay updated with the latest advancements in physical therapy techniques and contribute to the continuous improvement of clinical practices. Qualifications: • Valid licensure as a Physical Therapist (PT) in the state of New York., • Strong knowledge of physical therapy principles, assessment techniques, and evidence-based treatment interventions.
