Job Description Job Title: Community Health Programs Specialist Department: Community Health Programs Supervisor: Community Health Programs Manager Supervisory Responsibilities: None Location: Latham, NY Status: Full-time Qualifications: A bachelor's degree in Nutrition, Public Health, Health Administration, Social Work, or related field is required. Candidates should have a minimum of one year of experience in administrative or billing services and at least one to two years of experience in nutrition programming. Experience working with food access initiatives, Food as Medicine models, or nutrition assistance programs is strongly preferred. Familiarity with healthcare settings, including healthcare billing or insurance reimbursement processes, is also preferred. An interest in nutrition, food access, or community health is highly desirable. Must possess a clean NYS drivers license. Physical Requirements: This position works in a typical office setting but may be exposed to extreme temperatures in a warehouse setting. There may be extended periods of sitting, standing and/or bending, listening, talking and/or visual concentration, writing and/or computer use. This position would normally have routine opportunities for the employee to be able to alter their physical position or change eye focus. Occasional lifting, moving, and delivering boxes are required; lifting should not normally exceed 30 pounds. Position Summary: The Community Health Programs Specialist supports the implementation and daily operations of Medicaid 1115 and Food as Medicine services through billing, data management, partner coordination, and program support. This role prepares and reconciles billing and maintains accurate documentation to ensure compliance with Medicaid requirements, while tracking client enrollment, eligibility, service utilization, and outcomes for internal and external reporting. In collaboration with the nutrition team, the Specialist assists in planning clinically appropriate food box menus, develops educational materials, and provides nutrition education to patients to support healthy behavior change. The position coordinates with warehouse, procurement, and volunteer teams to ensure efficient food packing and distribution, communicates with healthcare and community partners, and assists leadership with program documentation, scheduling, and operational planning to ensure high-quality service delivery. Duties and Responsibilities: Billing, Reimbursement & Compliance • Prepare, submit, and reconcile billing and invoicing for Medicaid 1115 services in accordance with program requirements., • Maintain accurate financial and service delivery documentation for audit and compliance purposes., • Work with finance and program teams to track reimbursements and resolve billing discrepancies., • Track client enrollment, eligibility, service utilization, and completion of Medicaid 1115 services., • Maintain accurate databases and tracking systems for both internal and external reporting., • Generate required reports for funders, payors, and internal leadership., • Collaborate with nutrition team to plan food box menus that align with clinical goals, nutritional standards, and population needs., • Develop client-facing educational handouts in collaboration with nutrition staff., • Ensure materials align with program guidelines and client needs., • Communicate with healthcare, community partners, and internal teams regarding enrollment, service delivery, and program updates., • Plan and develop food box menus and pantry stocking orders accordingly for Food as Medicine partners., • Coordinate with volunteer team to manage the packing and distribution of food packages., • Maintain consistent communication with Food as Medicine partners and attenuate individual program needs., • Assist leadership with program documentation, scheduling, and operational planning.
