OVERVIEW The Refugee Health & Wellness Program focused on support, advocacy, and case management services for refugees with chronic or critical medical or mental health issues. The Refugee Health Promotion (RHP) case manager will provide health education, healthcare system orientation, and case management to newly arrived refugees in Albany and the Capital Region. This position reports to the Health & Wellness Manager and the Albany Field Office Director. ESSENTIAL DUTIES AND RESPONSIBILITIES • Provide direct case management services to refugees in the community under supervision of Health & Wellness Program staff., • Conduct home visits, intakes, and assessments of clients referred to the Health & Wellness Program under supervision of Wellness Program staff., • Coordinate appointments and services, and maintain case files and case notes for clients enrolled in the Wellness Program., • Communicate with healthcare providers about ongoing barriers or needs of clients served in the Wellness Program, and work to eliminate these barriers., • Provide education and orientation to small groups of refugees to support their healthcare literacy., • Work to connect clients served in the Wellness Program with supportive community networks and social support systems to ensure their long-term integration into the community and well-being. The EE I or II applies engineering principles to establish that the submissions are consistent with applicable technical standards and State and Federal regulations. This includes issuing comments, approvals or eligibility determinations to applicants. In addition, the EE I or II acts as the engineering project manager interacting with other internal divisions and government agencies, including but not limited to close coordination with New York State Department of Environmental Conservation and the New York State Department of Health. Essential Duties • Contact and establish working relationships with municipal officials and their engineers to determine need for, and interest in, EFC assistance for water quality and water pollution control projects. Based on information obtained from municipal representatives, present project priority and eligibility for CWSRF financial assistance, and participate and present to CWSRF scoring panel., • Review and make recommendations for approval or eligibility of engineering reports, plans, specifications, change orders, and other technical documents for the planning, design, and construction of CWSRF eligible water quality protection and wastewater treatment facilities and various point, non-point, and National Estuary projects. Coordinate the review of these documents with other interested State and/or Federal agencies for conformance with applicable technical standards., • Manage multiple projects at various stages of development from concept through construction completion by providing appropriate technical and application assistance to municipal representatives. Organize and participate in conferences with potential recipients of financial assistance to discuss program requirements and policies., • Proactively maintain and update project database to reflect the current status of the project and ensure program requirements (permits, approvals, etc.) are met. Develop and maintain spreadsheets and other project documents in MS Word and Excel to monitor project status., • Visit construction sites throughout the State to monitor construction activities, including observing construction for compliance with the program requirements, securing appropriate programmatic documents, and performance of interim and final project site visits on large and complex projects. Complete reports to record observations. In addition to the above duties, EE IIs may serve as a mentor to other engineering staff (EE I) and will be assigned more complex projects and tasks, and perform tasks with greater independence than EE I. Qualifications All applicants must possess a valid New York State drivers license as travel is necessary to conduct meetings and site visits in various parts of the state. Engineers are assigned to geographically based project sections and will typically handle projects within the regional area they are assigned to. However, candidates must be willing to work in, and travel to, any part of New York State as their regional assignment may require shifting due to staffing, workload, or other considerations. EE I Minimum required qualifications include a Bachelors degree in Environmental, Civil, or Chemical Engineering and successful passage of Part 1 of the National Council of Examiners for Engineering and Surveying Fundamentals of Engineering (FE) examination. Three years of full-time wastewater related engineering experience can be substituted for the FE examination. EE II Environmental Engineer II candidates must possess a New York State Professional Engineers (PE) license and current registration; and demonstrate the experience and judgement necessary to work independently at the EE II level, as well as the ability to provide mentorship/leadership as needed. At the Corporations discretion, years of engineering experience, Professional licensure or certification (EIT/PE) and/or a masters level degree in a related field may be considered when determining salary level. Knowledge/Skills Knowledge of methods, materials and terminology used in the planning, design and construction of water quality and wastewater collection and treatment systems; including the ability to understand, interpret and analyze technical material, such as engineering reports, construction plans and specifications, contracts, technical reference materials and instructions is required. JOB SUMMARY: This position works with nursing team to coordinate care for new patients. Monitors the ongoing home health nurse/patient relationship and patient progress. Directly involved in hiring, coaching, and counseling new field staff, provides orientation and educational training. The position will include establishing and maintaining positive relationships with patients, home health agencies and referral sources. Interacts with the intake department, clinical pharmacists, sales team, administrative departments, and outside agencies. Works with branch leadership to manage performance improvement program of the agency such as chart audits, formulation of Plan of Care (POT/485) and assessing staff’s clinical documentation technique, compliance with documentation guidelines and adherence to nursing standards. MAJOR DUTIES AND RESPONSIBILITIES: 1. Communicates effectively with a variety of people, both verbally and in writing., 2. Coordinates with intake department to obtain appropriate clinical information and coordinate nursing care for new infusion patients., 3. Assists with patient scheduling needs., 4. Serves as primary contact/liaison between KabaFusion branch, and various home health agencies., 5. Interacts with home health agencies and/or infusion agencies in providing authorizations for skilled nursing visits., 6. Develops cooperative relationships with nursing staff, home health agencies, referral sources, and KabaFusion accounting department., 7. Maintains knowledge of current state Home Health Regulations and other accrediting body standards; ensures compliance with both., 8. Ensures agency’s continuous compliance with the State Home Health Regulations and ACHC Standards in the operation of a home health agency., 9. Assists DON in creating an environment in which home health staff can work together, in which each has an opportunity for self-expression, for creating a sense of accomplishment and for personal growth., 10. Maintains adequate knowledge of documentation requirements in order to serve as resource for staff., 11. In collaboration with DON interviews and employs qualified personnel. Assures completion of staff evaluations. Performs disciplinary action as required., 12. Participates in providing orientation and clinical competency for new nursing staff, and field competency visits as needed., 13. Assist with patient home visits as needed and/or may carry a caseload., 14. Oversees day to day delivery of skilled nursing visits and agency operational functions., 15. Evaluates and intervenes in nursing-related patient care issues., 16. Assists DON in conducting staff meetings, in-services, and other meetings as assigned., 17. Reviews schedules for appropriate coverage., 26. Participates in coordination of on-call services during non-office hours as needed, performs “oncall” duties as needed or assigned., 27. BSN and/or Public Health Nurse Certificate preferred., • Current and unrestricted registered nurse licensed in the state of practice., • Active driver’s license and ability to provide current auto insurance, • Current BLS/CPR card, • Minimum of three years’ experience as a registered nurse in a home health agency, home infusion pharmacy or its equivalent., • Minimum of two years supervisory experience in a health care setting preferred., • Two years of supervisory experience in a health care setting required if mandated by state regulations.
