Job Description Position Summary: Supports psycho‐social needs of patients who are currently receiving or recently received hospital based care. The Social Worker strives to enhance the optimized level of health and/or recovery; provide interventions for populations ranging from infants to geriatrics; serve as a resource for the healthcare team, assists in identifying discharge planning needs; and provides support/intervention in stress filled situations. Experience: Minimum two years post graduate experience as a social worker in a similar type of health care setting is strongly preferred. Successful completing a 6 month hospital, based orientation process is required. For staff providing care to patients in the state of New York, LMSW, LSW, or LCSW is required. Essential Functions: • Provides psychosocial evaluation to meet patient's medically related social needs. Such services include, but are not limited to:, • Assessment of the social and emotional factors related to the patient's illness, his need for care, his response to treatment, and adjustment to care in the hospital;, • Assessment of the patients' medical and nursing requirements, home situation, financial resources, and the community resources available in making decisions regarding his discharge., • Participates in unit interdisciplinary conferences to identify patients and families need of intervention., • Identifies problems and develops plan based on evaluation of patient's needs, • Participates in an on‐call system 24‐hour accessibility and accountability to meet patient needs, • Receives training to allow the provision of care and services support resources to trauma patients., • Responds to consults as required by policy, • Facilitate discharge planning to meet the patient's needs for continued care (i.e. for skilled nursing, rehabilitation, acute care transfer, or other facilities), • Identifying psycho‐social, environmental and financial problems that affect discharge planning., • Collaborates with the healthcare team in establishing discharge plan, • Consults with Case Manager as necessary and requested, • Keeps patient/family/referring facility informed of plan, • Completes appropriate/required documentation related to transfers., • Completes documentation on patient status; every 24 hours for acute, every 48 hours for acute for patients with whom we are engaged, • Documents appropriately to reflect continuous progress in plan of care., • Plan of care is documented fully as required, • Content reflects adequate details to enable extended members of healthcare team to remain informed and implement plan, • Entries are timely and reflect significant interactions/interventions that occur, • Hand offs are documented fully by end of each business day, • Documentation consists of all required regulatory components, • Assess and facilitate financial needs, • Utilizes various charity funds available in a responsible manner according to policies/procedures, • Assists patients / families with financial applications (PAMA, NYMA, disability paperwork, etc.), • Collaborates with Case Manager, Patient Business office, etc. to facilitate meeting financial needs, • Support patient/family/others during crisis situations, • Provide intervention for trauma and other psycho‐social crisis situations throughout the hospital, • Assist Clinical Coordinator/others with crisis interventions when on‐call, • Provides interventions for identified high‐risk patients (i.e. abuse, neglect, substance abuse, etc.) Other Duties: Performs other duties as assigned. Job Description Position Summary: The Ophthalmic Technician ‐ Certified OR is responsible for performing advanced ophthalmic work up, procedures, measurements and tests to provide data to provider and assist in patient care. Assists provider throughout patient exam, office lasers and procedures, and performs as first assists in the operating room (minimum of 12 cases per year). Position requires advanced understanding of ophthalmology outpatient day to day office routine. The Ophthalmic Technician will continually demonstrate Patient Centeredness, Teamwork, and Excellence in the daily performance of their duties. This position requires the ability to be self‐motivated, flexible, punctual, detail oriented, have good time management skills and have excellent communication skills. Education, License & Cert: Certified Ophthalmic Technician (COT) preferred or Certified Ophthalmic Assistant (COA) with two years ophthalmic assisting experience; LPN with completion of home study course within first 6 months of employment. (Licensure in PA and NY required). CPR Certification preferred Experience: Surgical Technician Certified with OR experience may be substituted however completion of home study course within 6 months of employment and Certification as Ophthalmic Assistant is required within the first 12 months of employment. Employee will be paid at one grade lower than Ophthalmic Technician until certification is met. Candidates with clinical experience but no ophthalmic experience will be paid at the C5 pay grade until completion of home study course (3‐6 months); they will move to the C6 pay grade until they have successfully achieved a COA certification which is required within the first 12 months of employment. Candidates with ophthalmic assistant experience of 1 year will be paid at the C6 pay grade, they will be expected to complete home study course within 3 months if they have not already completed and they will move to the C7 paygrade once they achieve a COA certification which is required within the first 12 months. Completion of Ophthalmic Scribe Certification (OSC) is required within the first 3 weeks for those without COA, COT, COMT, LPN. Essential Functions: • Obtains and documents accurate chief complaint and comprehensive history of present illness. Reviews, verifies and updates patient’s past ocular, medical, family and social history. Reviews medical records for up‐to‐date master problem list including patient reported medication and allergies and diagnoses that are documented by physicians or providers., • Performs a portion of ophthalmic exam, ophthalmic diagnostic testing and procedures; documents findings/results on exam and stays current with new technology and procedures as they become available., • Performs as scribe for provider (throughout ophthalmic exam capture, summarize, & document pertinent examination data, clinical findings and the assessment and plan)., • Provides patient education, assists provider with informed consent, prepares and reviews surgical charts., • Assists provider with laser and surgical procedures within the office, including those that require aseptic and sterile technique., • Performs as first assistant in operating room., • Responsible for cleaning and sterilization of office instruments, performing basic maintenance, quality control, cleaning, disinfection and maintaining inventory of ophthalmic exam room and ancillary equipment according to organization and departmental protocols., • Responsible for documentation, tracking and reporting surgical co‐management; completion and processing of forms (DMV, disability, legal blindness, insurance, low vision, etc.); review and monitors provider’s schedule for efficiency and errors; triages messages and phone calls and responds to patients as directed by provider. Must have ability to react quickly to emergency situations such as patient fainting, patient being unsteady while standing for procedures, if unsafe situation occurs (such as fire), getting the patient to safety and sounding alarm. 9.
