Compensation & benefits: The good faith base annual salary range for this position is a minimum $75,000 to a maximum of $90,000. Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents.
