Job Description Arthritis Knee Pain Centers is looking to bring a dedicated Physician to our team! As a physician at Arthritis Knee Pain Centers, you will play a critical role in helping patients with joint pain and arthritis find relief through our cutting-edge treatments and therapies. You will be responsible for performing staff assisted viscosupplementation injections. The target is to ensure each patient experiences a comfortable and efficient visit each and every time. About the Role: The Family Nurse Practitioner (FNP) plays a crucial role in delivering comprehensive healthcare services to patients across various age groups and backgrounds. The FNP will collaborate with a multidisciplinary team to develop treatment plans, provide patient education, and promote wellness within the community. Minimum Qualifications: • Master's degree in Nursing with a specialization as a Family Nurse Practitioner., • Current and valid Nurse Practitioner license in the state of New York., • Experience in a primary care or urgent care setting., • Familiarity with electronic health record (EHR) systems., • Bilingual skills to effectively communicate with diverse patient populations., • Minimum 1 year experience as NP, • Graduated from approved Nurse Practitioner Program., • Licensed as Nurse Practitioner in New York State., • Basic CPR (BCLS) certification required., • Specialty Certification Preferred., • Conduct comprehensive health assessments and physical examinations for patients of all ages., • Diagnose and manage acute and chronic health conditions, including prescribing medications and treatments., • Educate patients and their families on health management, disease prevention, and wellness strategies., • Collaborate with healthcare team members to develop and implement individualized care plans., • Perform necessary procedures, including chemotherapy administration and urgent care interventions, while ensuring patient safety., • Involved with direct patient physical assessment, education, and health planning., • Assists in the coordination of services with other disciplines and patient’s insurance plans., • Advocates for patient in arrangement of medically necessary services., • Communicates with community’s resources to ascertain adequate assistance for patients in need., • Refers patients to specialty services when necessary., • Consults with collaborating physician and other members of the health care team to establish plan of care., • Participate in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of patients, • Maintains continuity of care by regular patient follow-up., • Participates in quality assurance process to assure optimal care delivery., • Provides comprehensive case management services that include emphasis on health promotion and disease prevention., • Monitors patient outcome in accordance with the practice protocols;, • Collaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planning., • Acts as a resource person for the nursing staff., • Conducts education in service on practice issues.
