Job DescriptionCase Manager - Caregiver Support & Respite Services Job Summary We are seeking a compassionate, organized, and mission-driven Case Manager to join a rapidly growing program dedicated to supporting family caregivers of the elderly population. This is an exciting opportunity to join a new and impactful initiative at the ground level and play a key role in shaping program operations, expanding community reach, and helping families access meaningful respite and support services. Working closely with the Program Director, the Case Manager will provide direct support to caregivers and older adults through assessments, care coordination, outreach, and resource navigation. The ideal candidate is empathetic, community-oriented, and passionate about improving quality of life for caregivers and aging populations. Schedule • Full-Time, • Monday-Friday, • 9:00 AM - 5:00 PM, • Structured daytime schedule supporting strong work-life balance Key Responsibilities Client Advocacy & Case Management • Manage an assigned caseload of family caregivers and elderly clients, • Conduct initial in-home assessments and ongoing re-assessments to evaluate caregiver needs and monitor respite services, • Provide ongoing support, advocacy, and follow-up to ensure quality service delivery Caregiver Support Plan Development • Develop individualized caregiver support plans using specialized intake assessments and available community resources, • Identify appropriate respite options and supportive services tailored to each family's needs Resource Coordination • Connect caregivers and families with community resources, public benefits, healthcare entitlements, and long-term care support programs, • Assist families in navigating complex service systems and accessing available support Program Growth & Outreach • Support outreach initiatives to recruit respite companions and expand community awareness of the program, • Represent the program at regional care councils, networking events, and community partnerships Training & Intake Support • Manage incoming calls and conduct initial intake screenings with prospective families, • Provide education and informational guidance regarding available services and program eligibility, • Assist with screening, onboarding, and training support for respite companions Operational Administration • Maintain accurate and timely case documentation within electronic database systems, • Process monthly client fee records and review/submit program timesheets, • Ensure compliance with organizational and program documentation standards Community Engagement • Coordinate caregiver awareness activities, educational workshops, simulations, and outreach events, • Promote caregiver wellness, support, and public awareness within the community Qualifications Required Qualifications • Bachelor's Degree in Human Services, Social Work, Psychology, or a related field, • Valid driver's license and reliable transportation for local travel, home visits, and outreach activities, • Comfortable with local travel up to 25%, • Proficiency with Microsoft Office Suite, email systems, and electronic database/case tracking systems, • Strong communication, organizational, and interpersonal skills, • Ability to work independently while collaborating closely with leadership and community partners Preferred Qualifications • Prior experience working with elderly populations and/or family caregivers, • Knowledge of senior resources, healthcare systems, public benefits, and community support services, • Experience in nonprofit, human services, or community-based program environments Why Join Us? • Opportunity to help build and shape a growing community-based caregiver support program, • Meaningful, mission-driven work that directly impacts families and older adults, • Collaborative and supportive leadership environment, • Opportunity for professional growth and community engagement, • Predictable weekday schedule promoting work-life balance If you are passionate about supporting caregivers, advocating for older adults, and helping families access critical resources and respite support, we encourage you to apply.
