Responsibilities • Coordinate patient care and facilitate safe, efficient transitions across the continuum of care., • Conduct utilization reviews to ensure appropriate level of care and resource utilization., • Develop and implement discharge plans that address patient needs and support continuity of care., • Collaborate with physicians, nurses, social workers, and other healthcare professionals to optimize patient outcomes., • Assess patient needs and identify barriers to discharge and care progression., • Communicate with patients and families regarding treatment plans, discharge options, and available resources., • Facilitate referrals to community resources, rehabilitation services, home care, and other post-acute care providers., • Maintain accurate and timely documentation within electronic health record systems., • Monitor patient progress and adjust care plans as necessary., • Participate in interdisciplinary care planning meetings and case reviews., • Ensure compliance with regulatory requirements, organizational policies, and quality standards., • Utilize independent clinical judgment to support patient-centered care decisions.Required QualificationsEducation, • Associate Degree in Nursing (ASN/ADN) from an accredited nursing program required., • Bachelor of Science in Nursing (BSN) preferred., • Master’s Degree in Nursing or related healthcare field preferred.Licensure & Certifications, • Active Registered Nurse (RN) License in the State of New York required., • Current Basic Life Support (BLS) Certification required., • PRI and Screen Certification required within three months of employment., • License verification through the applicable state licensing authority required.Experience, • Minimum five (5) years of clinical experience as a Registered Nurse required., • Experience in Care Coordination required., • Experience performing Utilization Review required., • Experience with Discharge Planning required., • Emergency Department, Case Management, or Care Management experience preferred.Skills & Competencies, • Strong knowledge of care management and case coordination principles., • Experience with utilization management and discharge planning processes., • Excellent clinical assessment and critical-thinking skills., • Strong communication, interpersonal, and patient advocacy abilities., • Ability to collaborate effectively within interdisciplinary healthcare teams., • Proficiency with electronic medical records and healthcare documentation systems., • Strong organizational, problem-solving, and time-management skills., • Ability to manage multiple priorities in a fast-paced healthcare environment., • High level of professionalism, accountability, and attention to detail.Additional Information, • Emergency Department setting., • Day shift schedule with no routine overnight responsibilities., • Opportunity to support patient-centered care and improve care transitions., • Collaborative multidisciplinary healthcare environment.
