Job Description Location: [Amityville, NY] Program: [ISCM] Hope For Youth is looking for a Full-Time Intensive Case Manager. If so, Hope For Youth wants you to join our dynamic Intensive & Supportive Case Management (ISCM) team! Hope For Youth's Intensive & Supportive Case Management (ISCM) program is an out-patient preventative service that provides children and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County. Key Responsibilities * Maintain a manageable caseload of 4 youth conducting twice-weekly home visits • Split time between our Amityville main office and the community with a flexible schedule, • Identify and address safety concerns and advocate for family needs, • Collaborate with interdisciplinary teams, Suffolk County Department of Social Services, and other community partners Complete all documentation and reports in a timely manner * Attend training and staff development opportunities provided by HFY, including but not limited to: Individual Supervision, Group Supervision, staff meetings, and webinars * Respond to all calls/emergencies via agency-provided mobile phone and/or via internet technologies How You'll Spend Your Time • Field work and home/community visits, • Documentation and case-related paperwork, • Staff meetings, supervision, and interdisciplinary team meetings, • Training and professional development, • Other duties as assigned To qualify for this job, you need to be or have a minimum: • Bachelor's degree in Social Work (BSW) or a related field, • Valid NYS Driver's License in good standing, • Reliable personal vehicle for regular home visits and transports throughout Suffolk County, • Strong writing and time management skills, • Flexibility to work evenings and occasional weekends, • Ability to respond to calls or emergencies via an agency-provided mobile phone or internet communication tools Physical Requirements • Ability to read and write reports/logs, • Prolonged walking, standing, or sitting, • Bending, stooping, kneeling, squatting, climbing stairs, and reaching overhead, • Repetitive wrist/finger movements for documentation, • Regular driving using a reliable personal vehicle This opportunity is being offered as Full time, 35-hour work week Monday – Friday (flexible as needed) Only candidates selected for an interview will be contacted.
