Job Description (Community & Facility Relationship Builder) Purpose • Generate referral relationships and in-person exposure by visiting healthcare and senior-care facilities across Nassau and Suffolk County Primary Responsibilities • Visit hospitals, specifically discharge planning and social work departments, • Visit rehabilitation centers and skilled nursing facilities, • Visit assisted living and independent living communities, • Visit adult day care centers and senior centers, • Introduce agency services to social workers, case managers, discharge planners, and activities directors, • Deliver marketing packets including flyers, brochures, and referral sheets, • Request and document the correct referral contact person, including direct email and phone number, • Schedule and confirm follow-up meetings (in-person or virtual) with decision-makers, • Attend local networking events including Chambers of Commerce, senior care events, and hospital vendor sessions, • Maintain daily visit logs, contact notes, and follow-up tracking, • Coordinate mailing of postcards through the United States Postal Service Scheduling Responsibilities (Light) • Schedule follow-up meetings with facility contacts, • Coordinate availability between agency and facility staff, • Maintain a weekly calendar of booked meetings and revisit dates Required Skills • Minimum 2 years of experience in sales, healthcare outreach, marketing, or a related field, • Professional communication suitable for healthcare environments, • Confidence speaking with hospital staff and administrators, • Ability to navigate clinical and professional settings appropriately, • Reliable transportation across Long Island, • Strong organization and follow-up discipline Deliverables / KPIs • 10–20 facility visits per week, • 3–8 qualified contacts collected per day, • 2–5 scheduled follow-up meetings per weekCompany DescriptionWe are a growing non-medical companion care agency dedicated to supporting seniors and individuals who need assistance with daily living.
