Our Day Habilitation Program is seeking a Program Manager at our Manhasset, Long Island location which serves about 30 individuals. JOB SUMMARY The Day Hab Program Manager is responsible for overseeing and managing the daily operations and services that are provided to approximately 35 adults. The Day Hab Manager is responsible for the development, coordination, implementation and monitoring of program services and the supervision of staff. EMPLOYEE BENEFITS • Low-Cost, High-Quality Healthcare Insurance PLUS a Supplemental Visa Cash Card to offset healthcare costs (ie. co-pays and prescriptions)., • Various Employee Discounts through our healthcare program for wellness and physical fitness programs (and others!), • Paid Time Off (Sick, Personal, Holidays) which advances work / life balance., • Life Insurance, • Dental / Vision Insurance, • Retirement / 403B Employer Match, • Paid training and on-site certifications, • An inclusive and diverse team environment, • Career Advancement Opportunities JOB RESPONSIBILITIES • Develop and design the individuals’ Program Staff Action Plans that complies with regulatory standards, and professional standards of practices., • Supervise, lead, coach and inspire Direct Support Professionals in the performance of all day-to-day activities and responsibilities., • Conduct job appraisals and disciplinary actions when needed., • Ensure that the individuals have a safe and clean environment, free of abuse and neglect., • Ensure community inclusion activities are based on Individual interests, preferences and choice with an ongoing dialogue with staff regarding options for community activities, development in general community integration and engagement and participation in community life., • Ensure individuals have the freedom to move from setting to setting, has access to community settings and choices regarding meals and meal times., • Maintain contact with families, referral sources and care managers for consultation on progress to ensure appropriate delivery of services to the individuals., • Provide ongoing in-services/training to all staff members, and introductory training and in-service to new employees to ensure appropriate delivery of services to the individuals., • Ensure that all Waiver Staff Action Plans compy with OPWDD regulations, • Review all documentation to ensure regulatory compliance and optimal quality outcomes., • Facilitate ongoing communication with the team members including, but not limited to the individual receiving services, Day Program Counselors, program nurse (when applicable), service Managers, family and residential providers (when applicable)., • Participation in the individual’s treatment team and service planning meetings. CORE COMPETENCIES • Excellent leadership and supervisory skills.
