Core Responsibilities: • Assistwith patient oversight ofChronic Care Management (CCM),Physical and Rehabilitation Medicine (PRM),Behavioral Health Integration (BHI), and other POD programs, • Monitor and review patient vital signs,alertsand other concerns, • Answers phones, respond to inquiries and triage calls appropriately, • Assistin the development and implementation of individualized care plans, • Respond to urgent careneedswhen necessary, • Assistwith administrative tasks, • Screen patients and make referrals accordinglyto other clinical lines, • Timely completion of clinical requests and follow ups, • Documentation audit reviews, • Maintaincomprehensive medical records of all patient interactions, • Perform other related duties as needed Work Schedule: Thursday, Friday, Saturday,Sunday and Monday 9am-5pm or Saturday, Sunday, Monday, Tuesday, Wednesday RequiredQualifications: • Graduate from an accredited Licensed Practical Nursing program, • High School Diploma or equivalent GED, • Current Licensure and registration asaLPN in New York State, • Excellent communication, interpersonal, organizational and time management skills, • Ability to work independently and collaboratively within a team environment, • Competency with EMRSystems andDocumentation, • Compassionate, patient- focused, and committed to providing high-quality care, • Knowledge ofcomplying withall state requirements and facility policies and procedures PreferredQualifications: • Current Licensure and registration asaLPN in New York StateandFlorida State, • Spanish Speaking
