Job Description Health and Recovery Plan (HARP) Care Coordinators will manage care for adults with significant behavioral health needs under the NYS DOH Health Home Model. They will facilitate the integration of physical health, mental health, and substance use services for individuals requiring specialized approaches, expertise, and protocols which are not consistently found within most medical plans. In addition to the State Plan Medicaid services offered by Mainstream MCOs, qualified HARPs will offer access to an enhanced benefit package comprised of Home and Community-Based Services (HCBS) designed to provide the individual with a specialized scope of support services not currently covered under the State Plan. Essential functions include: Job Summary Gather information for and or conduct outreach, intake, assessment, and reassessments. Provide care management to a caseload through the coordination of medical, mental health, HCBS and substance abuse services. Conduct assessments and prepare a comprehensive plan of care as directed by NY State and Managed Care Organizations. Collaborate with the individual's HARP team including: MCOs, HCBS providers, as well as other medical and treatment providers. Monitor client's progress in utilizing services (appointments, treatment, medication, etc.) through telephonic and direct contact. Check that clients receive test results and track that clients follow up with medical directions. Escort clients to/from any appointments when needed. Document service information in databases and use electronic systems to track participant activities. Assist in crisis intervention and provide or refer to crisis services. Extensive field work required, including home visits and community work such as visiting hospitals and emergency rooms when determined necessary. Ensure that clients follow-up with aftercare discharge (i.e. fill prescriptions, make appointments). Job Description JOB DEFINITION: The Psychiatric Rehabilitation Care Coordinator (PRCC) functions as a member of a multidisciplinary treatment team to provide skill-building, vocational, and care-coordination services to individuals diagnosed with mental health and/ or substance use disorders. The PRCC partners with clients to identify social, community, and skills-based needs, and works with individuals to develop skills needed for meeting those needs. This role directly serves clients, engages with clinical treatment team members, and advocates with community-service agencies to reduce reliance on the behavioral health system, increase wellness self-management, and promote recovery.
