We invite you to take your first step toward a rewarding career with us! \n Key Responsibilities: \n\n • Provide outstanding customer service to all residents, family members, responsible persons, and visitors to the residence\n, • Coordinate and deliver nursing care to residents within the scope of an LPN, including but not limited to catheter care, wound care, dressing changes, nebulizer treatments, ostomy care, and management of medical equipment; medication management including administration of oral medications, injections or PRN medications.\n, • Supervise the care provided by HHAs and RCAs to residents.\n, • Manage and administer medications and assist the residents with self-administration of medication.\n, • Maintain positive collaborative relations with residents, families and provider partners\n, • Report any observed significant incidents and/or changes in the resident’s condition to Nursing Leadership (Charge RN in the Residence), Primary Care Provider and family as indicated.\n, • Review and become familiar with each assigned resident’s Individualized Service Plan.\n, • Assist in training and orientation of Resident Care staff.\n, • Document care and services provided to residents in the electronic health record using nursing best practices and documentation standards.\n, • Follow all Inspīr policies and procedures as outlined in the policy and procedure manual\n, • Attend and participate in scheduled in-service programs, training programs and staff meetings.\n, • Assist with scheduling and staffing of Resident Care staff as needed.\n, • Provide coaching and mentoring to Resident Care Team members as needed.\n, • Follow emergency procedures.\n, • Understand and comply with all Inspīr safety practices and procedures.\n, • Participate in health, wellness and lifestyle programs and assist with all aspects of resident care and resident experience as needed\n, • May perform other duties as assigned.\n\n \n Education/Experience/Licensure/Certification: \n\n • Must be a Licensed Practical Nurse, licensed to practice in the state of New York with one of the following:\n, • Associates degree and a minimum of two (2) years of full time, or the equivalent of full time, clinical experience in nursing, at least one (1) year of which shall be in a home health care agency, hospital, skilled nursing facility, or community health program that included care of the sick at home\n, • A diploma or associates degree in nursing and at least four (4) years of full-time, or the equivalent of full-time, clinical experience in nursing within the past ten (10) years, at least one (1) year of which shall be in a home health care agency, hospital, skilled nursing facility, or community health program that included care of the sick at home\n, • Basic first aid and CPR certification.\n, • Experience in assisted living, post-acute or long-term care setting preferred.\n, • Competitive wages\n, • Flexible shifts\n, • Growth opportunities\n, • Bonus & incentive programs\n

Job Description Wellness Nurse - LPN - Full Time Days and Evenings Salary Range: $33+/HR Led by the community's Health and Wellness Director (RN), our Wellness Nurse is a Licensed Practical Nurse (LPN) who provides direct nursing care to the Residents with an emphasis on holistic wellness. They also help supervise the day-to-day nursing activities performed by care staff of Certified Nursing Assistants. Schedule: The Wellness Nurse (LPN) is Full-Time with two shifts available: 7am-3pm or 3pm-11pm Responsibilities and Duties • Monitors the health, safety, and well-being of all residents, • Assisting in training and monitoring of medication administration, • Supervise care staff in accordance with current state regulations and community policies, • Maintaining clinical quality assurance in accordance with federal, state and local standards, • Promote the highest degree of service to our residents while leading and demonstrating the mission of the company, • Conduct thorough resident assessmentsQualifications, • Graduation from an accredited School of Nursing, with current LPN license, • At least 2 years’ experience with long-term care, assisted living, home health or hospital setting preferred, • Experience working with residents with Alzheimer’s or other related dementias, • Ability to handle multiple tasks, • Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to health and wellness resident, • Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications, • While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell., • An individual in this position will be required to lift or carry weight in up to 50 lbs.
