Job DescriptionWe are currently seeking a Registered Nurse Case Manager to join our rapidly growing team to oversee Nassau County, Suffolk County and Queens County. At Affirmed Home Care, we believe in compromising nothing when it comes to providing exceptional, client-centered care. This philosophy extends not only to our clients, but to our staff as well. As the RN Case Manager, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client’s ongoing needs in a private pay, concierge level home care. This role requires 70% fieldwork for a small caseload of clients demanding great attention to detail. The salary for this full-time position ranges from $110,000 to $130,000, commensurate with related experience.
