Major Duties: • Maintain primary case planning responsibility for 16-20 cases in collaboration with the social worker., • Establish immediate contact with families to provide information about the program., • Conduct required casework contacts with the child, parent, and collateral contacts (schools, service providers, medical personnel, mental health practitioners, youth development counselor and substance abuse counselors)., • Work collaboratively with Social Worker to make assessment-based referrals as necessary to meet the needs of the child and/or parent., • Assess birth parent’s homes for diagnostic purposes to ensure the safety of youth and family while at home and complete home safety assessments., • Attend court and permanency hearings., • Maintain up to date case records within agency standards and complete all necessary ACS/Agency documentation including FASPs, Progress notes, Permanency Hearing reports and all other reports necessary to fulfill the function of the work., • Progress notes are to be completed and submitted preferably within one day of the event but no later than the end of the given week of the event., • Submit, PYA, FASP and Permanency Hearing Report on the internal date for review provided by the agency's compliance department., • Manage case load of AWOC and work collaboratively with assigned Social Worker to locate AWOC youth., • Develop community relationships and communicate with all collaterals, including ACS., • In collaboration with the primary social worker, youth development counselor, work with full treatment team, family and youth to develop the most appropriate permanency plan with emphasis on lasting family connections and/or other permanent caring connections in the community., • Attend and participate in treatment team meetings, service plan reviews and FTC’s; provide relevant input., • Maintain appropriate and relevant documentation of all contacts and events., • Effectively use and practice the Sanctuary Model tools such as safety plans, community meetings, psycho-education groups, team meetings, red flag reviews, treatment planning conferences and self-care in the department/program to create a non-violent and trauma-sensitive culture., • Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma sensitive culture., • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) Framework into daily practice to ensure quality care and an effective work environment., • Provide coverage for Marian Hall cases when necessary., • Bachelor’s degree required., • Experience working with families and adolescents in the child welfare system required., • Must be able to establish working relationships with participants in need of supportive services., • Must have working knowledge of Connections., • Must have excellent verbal and written communication skills., • Ability to work flexible hours, including two evenings., • Ability to use public transportation to travel throughout New York City., • Must have a commitment to GSS models of practice including Sanctuary and ACS principles to promote permanency, safety and well-being., • Must have a commitment to work from a strength-based, youth development and trauma informed perspective., • Bilingual English/Spanish preferred. Responsibilities Summary Under close supervision, this position provides support and structure to residents in developing program plans by managing a caseload of client cases and administering the policies and procedures of the program as directed by the contracting agency and the Facility Director. Learns, uses and models the principles of the “What Works” system through evidence-based practices in all interactions with clients and other staff members. Trains and assists less experienced case managers. Primary Duties and Responsibilities • May perform the duties of a Monitor including resident intakes, check-ins, breath analysis and drug screens, to provide backup when necessary., • In own handwriting, prepares written personalized program for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals., • Reviews plan with residents on a weekly basis and document progress and/or revisions, so resident is gaining the life skills necessary to make a successful re-entry into the community., • Assists residents in establishing personal budget, locating suitable housing, enrolling in educational and vocational programs, and participating in family and community activities, as appropriate, to ensure that they are working towards financial independence., • Monitors facility sanitation and safety. Assigns daily housekeeping chores to residents to comply with facility standards., • May orient residents upon arrival at facility to provide smooth assimilation into program (e.g. have resident complete paperwork, take on facility tour, introduce to other residents, etc.)., • Makes entries in own handwriting into confidential client (resident) files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensures files are secure to protect client confidentiality., • Assists Employment Placement Specialist to verify residents' jobs to ensure residents' employment is suitable under established program plan. Counsel unemployed residents by providing guidance in obtaining employment in cooperation with the Employment Placement Specialist., • Verifies residents' pass plan through pass sponsorship according to contract, so that the resident is accountable for his/her whereabouts in the community., • Prepares terminal report within established time parameters upon resident release to summarize program performance and to satisfy contract requirements., • Participates in staff meetings by sharing observations and recommendations regarding the facility, residents, and staff relations to identify and solve problems and function as a unified team., • Maintains confidential information obtained through job duties regarding employees, vendors, residents, outside agencies, etc., so sensitive information is on a "need to know" basis., • Complies with GEO policies and work rules as described in the company Employee Handbook, and the Facility Policies and Procedures Manual, including directives of GEO managers and supervisors in a cooperative spirit to ensure conformance to company standards., • Adheres to GEO’s attendance policy; be available for all shifts; work overtime as required to ensure adequate coverage of the program and to ensure the safety and security of the residents., • Complies with facility safety rules; take appropriate corrective action to ensure work is performed in a safe manner and without injury to self or others., • Communicates effectively with all levels of staff in a manner that promotes a team spirit and a sense of cooperation., • Performs other duties as assigned. High School diploma or GED plus two years full time experience providing direct services to individuals with mental disabilities or Associates degree (or currently matriculated in a 4 year degree program and completion of half the course requirements) plus 2 years full time experience providing direct services to individuals with mental disabilities or Bachelor’s degree plus 1 year experience providing direct services to individuals with mental disabilities. Minimum experience requirements are waived for candidates with a bachelor’s degree that required a one-year practicum working directly with individuals with mental disabilities (e.g., BSW).
