Job Description Position Overview As a critical member of our primary care team, the Nurse Practitioner will deliver high-quality, compassionate healthcare to patients of all ages. This role involves diagnosing and managing acute and chronic conditions, developing personalized treatment plans, and promoting disease prevention and healthy living—all within an interdisciplinary environment. Key Responsibilities • Conduct comprehensive physical exams and health assessments for patients across the lifespan (infants to older adults)., • Order, interpret, and act on diagnostic tests and laboratory results., • Diagnose, treat, and manage both acute and chronic illnesses; prescribe medications and launch therapeutic interventions as needed., • Create and oversee individualized treatment plans, adjusting as necessary based on patient response., • Provide preventive care and health education, including screenings, immunizations, wellness promotion, and lifestyle counseling., • Maintain accurate and timely clinical documentation in the electronic health record (EHR) system, including patient histories, encounter notes, prescriptions, and referrals., • Coordinate care with specialists, manage referrals, and ensure seamless transitions across the continuum of care., • Collaborate effectively with clinical teams—nurses, medical assistants, physicians—to optimize patient outcomes., • Participate in quality improvement initiatives and support the training of students, residents, or new clinicians as applicable. Minimum Qualifications • Master’s degree (MSN) or Doctor of Nursing Practice (DNP) from an accredited NP program ., • Active Nurse Practitioner license in New York State; current RN license., • National NP certification (e.g., AANP or ANCC)., • DEA registration (for prescribing medications) and BLS certification ., • Preferred: 1–2 years of experience in ambulatory primary care or related patient care setting., • Will consider the right New Grad, • New Grads with strong nursing history will be considered Preferred Skills & Attributes • Strong clinical judgment and critical thinking skills., • Excellent verbal and written communication abilities with diverse patient populations., • Proficiency with EHR systems and practice management tools., • Ability to manage time effectively in a fast-paced environment., • Commitment to patient-centered care, cultural competency, and health equity.
