As a team member of the SPOA unit you would be processing applications for adults with mental illness seeking an array of services, including but not limited to: Assisted Outpatient Treatment, Case management and ACT services, SPA Mental Health Housing, and/or Crisis Respite housing. This requires thorough review of the application and supporting documentation; investigation of documents; case presentations; general knowledge regarding Health Homes and resources in the mental health system; general knowledge of mental health housing in Suffolk County; ensuring court-ordered Mental Health Services are in place and that the person under an AOT order is compliant with those services. In addition to linking individuals to needed services, you will also have a caseload of individuals under an active AOT court mandate, which you will be responsible for receiving, documenting and following up on weekly updates from current providers, as well as requesting and following up on additional documentation as required. This is a very intensive program, where the work can, at times, take on an emergent need. You will need to be organized and able to complete tasks timely and efficiently. Job Description RiseWell Community Services is seeking a Full-Time Dual Recovery Specialist providing services to clients residing in a licensed residential program in Sunken Meadow. RiseWell’s Residential Services provide those in recovery from serious mental illness a safe, reliable home with on-site support. Our Residential team assists participants with an array of services aimed at supporting their recovery and improving their quality of life. Services include medication monitoring, skill-building development, symptom management, socialization, transportation training, connections to psychiatric and medical services, and more. Job Description RiseWell Community Services is seeking a Part Time Assistant Case Manager who will report to the Site Supervisor within the Outreach Services Department at Pilgrim. In this role, you will assist Case Managers by providing selected services to ICM clients. Our Outreach programs provide an array of services designed to engage individuals who are reluctant to seek help or experience barriers to access. Services are brought to the person where they are located, whether that be at home, the street, a hospital, or another location. As a part of this team, you’ll assist those we serve to have their basic needs met by providing referrals, linkage, and hands-on assistance with obtaining food, employment, housing, and financial assistance.
