Job Description Position Description The Patient Navigator is responsible for providing outreach, engagement, and patient navigation services to individuals who are contacted through the SAMHSA program. They will conduct screenings, service orientation and refer individuals internally and externally to appropriate community resources for needed services. They will also enroll eligible individuals into care by navigating and enrolling them into the SAMHSA program and additional services as needed, linking them to our community-based HIV/HCV testing, as well as medical care at Harlem United Health Services as indicated. The Patient Navigator will also connect clients to appropriate case management, substance use services, benefits enrollment, and supportive services throughout the agency. They will conduct outreach (phone, field) to clients who have been lost to care/follow up and establish ongoing relationships to ensure successful connection to quality medical and social services. Essential Job Functions The following duties are mandatory requirements of the job: Peer Supervision • Supervision of Peer Educators who work with SAMHSA team by conducting intakes and assisting with administrative needs of the program. Conduct preliminary interviews/screening for program to assess client’s immediate needs, including medical, insurance, case management, mental health, housing, treatment adherence, and other supportive services., • Provide referral services to at least 15 clients per month. Referral services involves the implementation of a brief service plan leading to successful connection to harm reduction and substance use services, including substance use treatment, medication assisted treatment, detox programs, outpatient services, groups, or other support services. Verification of referral services is required of all clients. Completion of service verification is required for all clients., • Complete all necessary programmatic paperwork including assessments, programmatic intakes and reassessments, and service plans., • Ensure the maintenance of appropriate, accurate, and clear documentation of all outreach, education, and client activities, including chart documentation and progress notes., • Complete data entry for client services into required online systems as indicated by funder and by internal Harlem United documentation standards., • Through continuous quality improvement activities, ensure that all clients referred to medical services are connected to care and attend their first medical visit. Assess client need for additional services and ensure that those referrals are provided and attended., • Distribute harm reduction materials including safer sex kits, educational literature, and brochures., • Assist in the guidance of the program’s peer educators, providing assistance for their work with clients in the field., • Perform other duties as assigned. Job Description Mitsubishi Electric Trane HVAC US LLC is looking for a Sr Specialist, Regional Marketing in New York, New Jersey, Philadelphia regions. POSITION SUMMARY: The Sr Specialist, Regional Marketing is responsible for a variety of marketing activities focused on increasing sales of new and current Mitsubishi Electric Trane HVAC US (METUS) customers. They will work directly with the Regional Marketing Management on key initiatives including assessing dealer lead and marketing efforts to develop and implement improvement strategies, and contractor and distributor marketing tool support. ESSENTIAL FUNCTIONS The essential functions of the position include, but are not limited to the following: • Partners with Business Unit contractors, sales staff, and distributor staff to optimize lead management., • Prepares weekly distributor lead reports, and works with dealers, distributors, and BU sales staff to improve lead response and closure rates., • Plans and execute campaigns in conjunction with METUS Corporate Marketing team, outside agencies/contractors, and content developers., • Determines marketing campaign goals, establishes media plan, develops messaging and assets, and rolls out campaign to METUS Business Unit sales team, distributors, and dealers., • Measures campaign success and reports results to management team and Business Unit team., • Increase dealer, distributor, and Area Sales Managers’ utilization of effective sales tools., • Create, implement, and assess the effectiveness of localized tools and marketing assets / content (in conjunction with METUS corporate marketing team or locally sourced contractors)., • Plan and support local events to improve effectiveness of dealers and distributors in marketing METUS products., • Develop and deliver educational content to Business Unit sales staff, Diamond Contractors and Ductless Pros, and Distributor staff to educate them on HVAC / METUS marketing best practices and programs., • Willing to travel up to 40% to for various Company meetings and training both domestically and possibly internationally., • Bachelor’s degree in Marketing, Business, creative tools/digital art or related field., • 3+ years progressive experience in a marketing support role., • 3+ years in customer-facing relationship management, customer training, account management role preferred., • 3+ years progressive experience managing projects involving cross-functional teams., • Demonstrated ability to work well with all levels of internal management, staff and outside clients and vendors., • Basic to proficient with PowerPoint, Excel, Word, and other Microsoft Office products., • Proficient in Content Creation programs (Adobe Creative Suite or similar), project management tools (Jira or similar), Digital Asset Management (DAM) and other marketing related software programs., • Familiar with salesforce, a plus., • Demonstrated interpersonal communication skills, both verbally and written.
