We are a Federally Qualified Health Center (FQHC) that provides Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. We are a multi-specialty healthcare provider caring for our community through Primary and Specialty Medical Care, Dental, Behavioral Health, Audiology, Physical Therapy, Occupational Therapy and Speech Language Pathology. About the Role: As a Licensed Practical Nurse (LPN) at Long Island Select Healthcare Inc., you will play a crucial role in providing high-quality patient care in a supportive and compassionate environment. Your primary responsibility will be to assist registered nurses and physicians in delivering medical care, ensuring that patients receive the attention and treatment they need. You will be involved in monitoring patients' health, administering medications, and documenting their progress, which is essential for effective treatment plans. Additionally, you will educate patients and their families about health management and wellness, fostering a collaborative approach to healthcare. Ultimately, your contributions will significantly impact the overall well-being of our patients and the efficiency of our healthcare team. I. General Purpose of the Job To provide direct nursing care under the supervision of a registered nurse, nurse practitioner or physician II. Essential Duties & Responsibilities 1. Review daily schedule and ensure set up of medical charts each day., 2. Daily thorough cleaning of rooms as well as cleaning between patient turnovers., 3. Obtain vital signs (blood pressure, pulse, temperature, respiration) at each appointment., 4. Obtain height and weight measurements as needed, 5. Collects blood samples via venipuncture and processes for lab testing, as required, 6. Performs routine urine sampling via dip sticks and collection of urine samples for culture and sensitivity testing as required, 7. Performs EKGs as ordered by supervising physician/nurse practitioner, 8. Assists physician(s) and advance practice professionals (ie: NP, PA) with direct patient care as needed, 9. Assists in Gynecology clinic and prepares Pap slides for lab testing, 10. Prepares and sterilizes instruments., 11. Processes medical records, as required, 12. Manages and maintains waiting room, office and exam rooms, as needed, 13. Obtains Authorization for prescription medications, procedures, etc. as needed, 14. Triage telephone calls, 15. Maintains accurate and orderly log records of lab specimens taken., 16. Follows up pending labs, x-rays, testing, etc., 17. Maintains office supplies and inventory and orders all medical and lab supplies, 18. Instruct patients as needed regarding procedures, self-care, office hours, etc., 19. Will provide relevant information about the PCMH program including brochures, that describe the values and goals of the program to patients. The information disseminated will be based upon the person’s role as it relates to patient care within LISH, 20. Confirms appointments as needed, 21. Implements ICD-10 and CPT coding for insurance processing, 22. Applies computer concepts to office practice, 23. Monthly inspections of all emergency equipment and supplies, 24. Routine inspections of expiration dates on all supplies, 25. Schedules appointments and perform duties as assigned by the Director, Medical Director and/or Nurse Practitioner to support and ensure general medical operation of the Health Center, 26. Agency Infection Control Committee participation at monthly meetings, 27. Participates in maintaining safety of work environment, 28. Adheres to Agency’s policies, procedures, rules and regulations of confidentiality III. Supervisory Responsibilities None IV. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience. New York State licensure as a Licensed Practical Nurse. Certified in CPR. VI. Necessary Skills & Abilities Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Ability Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Computer Skills To perform this job successfully, an individual should have basic to intermediate knowledge of Microsoft Office software (Outlook, Excel, Word), as well as Practice Management and Electronic Medical Record software.
