As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct annual in-home health assessments for Medicare Advantage and other plan members.

Provide direct patient care, within the limits prescribed by training, and in consultation with a supervising physician. Multi speciality office, primarily Internal Medicine and Cardiology. Prepares patients for physician visit and examination assisting as directed.
