Notify all participants including outside collaterals of the meeting in a timely fashion., • (cooking, arts & crafts, trips, etc.) and familiarize other staff with any changes to ongoing individuals’ plan, • Accompany individuals to community activities (including volunteer experiences) and on trips. Carry out pertinent methodologies with individuals while in the community., • Maintain and thoroughly clean equipment and supplies, • Carry out assignments and/or activities as directed by the professional/supervisory staff., • Assist with transporting consumers to/from the Day Habilitation Program, to/from their community placements, and to/from other activities as directed., • Act as the liaison to parents, residences, MSC’s, etc. Ensure that they receive pertinent information. The Ryan White Support Services program addresses unmet social service needs that have significantly hindered their access to and engagement in HIV and other essential medical care. This program will expand services to Staten Island and establish community-based, non-Medicaid eligible services in Queens to assess, identify, and address unmet social determinants of health (SDOH) needs. The Community Navigator (Queens) will be responsible for delivering non-medical case management and food nutrition education to clients enrolled in our second Ryan White Support Services program, located at Sun River Health’s Sutphin Health Center in Jamaica, Queens. The Community Navigator will be trained on NYSDOH-approved HIV Health Education/Risk Reduction curriculum, food and nutrition counseling/education, and developing an individualized Healthy Food Security Plan for clients.
