As the Clinician, you will play a pivotal role in individuals' lives, making a significant impact through: • Conduct clinical assessments, including diagnoses and recommendations, to inform treatment planning., • Conduct intake interviews with prospective residents, family members, and referral sources, presenting applications for review when instructed., • Develop and review treatment plans and Individual Crisis Management Plans with clients during bi-weekly treatment meetings and supervisory sessions., • Conduct advocacy casework, including foster care, CPS, Family Court, Medicaid, Public Assistance, mental health, and substance abuse coordination., • Maintain comprehensive case files, documenting progress toward treatment goals, casework contacts, FASPs, and educational/health activities., • Complete clinical treatment summaries regularly and final discharge reports for the local Social Services District.
