Job Description In the context of child-centered, the Case Planner provides family-focused practice to promote permanency, safety and well-being, and to reduce length of stay in foster care, the case planner also provides comprehensive casework services for both youth and families. The case planner provides all case management services and ensures that all ACS mandates are in compliance. Casework delivery is to be consistent with Good Shepherd Services’ philosophy and within city and state mandates. Please note this position is based ONSITE with the possibly of 1 or 2 days Remote. We provide a range of programming for Vulnerable Youth/Adolescent with history of Trauma. Job Title: Case Planner, Marian Hall Reports to: Assistant Director of Social Services Location: 337 E. 17th Street, New York, NY 10003 Hours: 40 Hours/Week; Non-Exempt Position requires staff to be onsite along with some field work. Some evening will be required. Hybrid Schedules are based on program needs and at leadership discretion. Major Duties: • Maintain primary case planning responsibility for 16-20 cases in collaboration with the social worker., • Establish immediate contact with families to provide information about the program., • Conduct required casework contacts with the child, parent, and collateral contacts (schools, service providers, medical personnel, mental health practitioners, youth development counselor and substance abuse counselors)., • Work collaboratively with Social Worker to make assessment-based referrals as necessary to meet the needs of the child and/or parent., • Assess birth parent’s homes for diagnostic purposes to ensure the safety of youth and family while at home and complete home safety assessments., • Attend court and permanency hearings., • Maintain up to date case records within agency standards and complete all necessary ACS/Agency documentation including FASPs, Progress notes, Permanency Hearing reports and all other reports necessary to fulfill the function of the work., • Progress notes are to be completed and submitted preferably within one day of the event but no later than the end of the given week of the event., • Submit, PYA, FASP and Permanency Hearing Report on the internal date for review provided by the agency's compliance department., • Manage case load of AWOC and work collaboratively with assigned Social Worker to locate AWOC youth., • Develop community relationships and communicate with all collaterals, including ACS., • In collaboration with the primary social worker, youth development counselor, work with full treatment team, family and youth to develop the most appropriate permanency plan with emphasis on lasting family connections and/or other permanent caring connections in the community., • Attend and participate in treatment team meetings, service plan reviews and FTC’s; provide relevant input., • Maintain appropriate and relevant documentation of all contacts and events., • Effectively use and practice the Sanctuary Model tools such as safety plans, community meetings, psycho-education groups, team meetings, red flag reviews, treatment planning conferences and self-care in the department/program to create a non-violent and trauma-sensitive culture., • Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma sensitive culture., • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) Framework into daily practice to ensure quality care and an effective work environment., • Provide coverage for Marian Hall cases when necessary., • Bachelor’s degree required., • Experience working with families and adolescents in the child welfare system required., • Must be able to establish working relationships with participants in need of supportive services., • Must have working knowledge of Connections., • Must have excellent verbal and written communication skills., • Ability to work flexible hours, including two evenings., • Ability to use public transportation to travel throughout New York City., • Must have a commitment to GSS models of practice including Sanctuary and ACS principles to promote permanency, safety and well-being., • Must have a commitment to work from a strength-based, youth development and trauma informed perspective., • Bilingual English/Spanish preferred. The Social Service Supervisor will provide supervision to Case Managers using Evidence Based Practices (EBP), Motivational Interviewing (MI), Trauma Informed Care (TIC), and boundary setting. Responsibilities: * Provide comprehensive case management including individual and group counseling services and other appropriate support services to clients in the absence of the Case Managers. * Supervise Case Managers and provide coaching, training, and staff development as needed. * Manage the shelter’s multidisciplinary team. * Champion the use of Evidence Based Practices (EBP) in all aspects of Win services. * Use and promote EBP, MI, MIC, and TIC to exceed program goals. * Assume responsibility for Program Services in the absence of the Program Director. * Participate in DHS daily housing metrics calls. * Provide crisis intervention, oversee day-to-day case management services and ensure weekly case conferences. * Ensure all Case Managers conduct an initial needs assessment with new families within 48 hours of arrival at the shelter and begin to develop an Individualized Living Plan (ILP). * Participate in Child Safety Conferences and provide information as a Mandated Reporter to the Administration for Children’s Services (ACS) as needed. * Ensure Case Managers are provided with orientation and training related to their responsibilities in maintaining the Uniform Client Chart. * Ensure Case Managers maintain current and accurate confidential case records with all required documentation. * Audit the Client Assistance and Rehousing Enterprise System (CARES) database, review essential documentation, and monitor chart maintenance for OTDA, DHS, and Win audit readiness. * Work with community service providers to ensure clients receive essential services including mandated substance abuse treatment and mental health services. * Attend regularly scheduled CARES training sessions to obtain DHS updates. * Develop and conduct client activities/workshops. * Schedule and attend client monthly house meetings. * Provide feedback to the Program Director to ensure resources are available and the goals of the program are achieved. * Prepare accurate and timely reports required by funders and Win. * Perform other related duties as needed. Our Mission: Safe Horizon's mission is to provide support, prevent violence and promote justice for victims of crime and abuse, their families and communities. Safe Horizon is the nation’s leading nonprofit victim assistance, advocacy and violence prevention program. Safe Horizon’s Child Advocacy Centers (CAC), play a critical, front-line role in responding to child sexual abuse and severe physical abuse by providing a coordinated and collaborative approach to the investigation of such cases, while ensuring that victims receive immediate, effective support. In each Child Advocacy Center, Safe Horizon partners with the New York Police Department, the Administration for Children’s Services, the local District Attorney and local medical providers (currently from Brooklyn Hospital Center, Cohen’s Children’s Hospital and Columbia University Hospital), to ensure that an expert response begins as soon as a child victim steps through the door. At the Safe Horizon CACs in Brooklyn, Queens, Staten Island, and Manhattan a team of child protective caseworkers, police detectives, pediatricians, prosecutors and Safe Horizon staff work together in a child-friendly setting to conduct joint forensic interviews and provide expert evaluation and therapeutic services, in order to improve the quality of child abuse investigations, prosecutions and outcomes and minimize further trauma to child victims and their families. Essential Responsibilities: • Overseeing the answering and directing telephone calls, faxes and mail directed to the person or department, • Ensuring that the client reception area is supervised at all times, • Acts as liaison with MDT partners for purchasing supplies, processing paperwork, • Supervising the Program Assistant/Volunteers and Interns providing reception area coverage, • Ensuring that reception staff link all families to the appropriate CAC service providers, • Collecting basic client intake information, • Assisting in straightening up the playroom at the end of every day, • Overseeing the Centers’ administrative duties including assisting with report preparation, supply ordering, petty cash reconciliation, NCAtrak data input, processing payment requests, • Scheduling and coordinating internal and external meetings, • Participating in weekly supervision, staff meetings and required trainings, • Proficient with MS Office suite (Word, Excel, Powerpoint, Outlook), • B.A. preferred or equivalent combination of college course work and professional experience; Excellent interpersonal and organizational skills required.
