Job Summary: The Case Planner provides comprehensive case management services for all youth who reside in their respective Site i.e. Non-Secure Placement or Limited-Secure Placement facility and their families. The position is demanding and highly interactive with the youth and their families, staff and community. Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin. The Case Planner will maintain a caseload of 15 youth both in the facility and in the community on aftercare. The role of the Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Case Planner is responsible for developing and maintaining meaningful contact with youth, their families, and service providers for medical, mental health, social, educational, and other related service needs. Since 1996, we’ve worked alongside communities, courts, and those most directly affected by the justice system to build stronger, healthier, and more equitable neighborhoods. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks Case Managers for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Managers, Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, Case Managers will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. Responsible for the accuracy of data entered into a computer database or manual records and assists the Program Director in collecting, organizing, and submitting client information, e.g., CAIRS, ASFA., • Informs staff of the types of clinical and rehabilitative services, recreational activities, and ICL programs available., • Reviews crisis assessment of clients with staff, reviews nonverbal and verbal crisis intervention techniques to be used with each individual client, and staff actions during a crisis situation., • Informs staff of community resources and ensures that staff are accompanying clients on regularly scheduled or emergency visits to medical treatment facilities, social agencies, government offices, or other locations associated with treatment or assistance of the client., • Immediately reports serious incidents, allegations of abuse, incidents, or sensitive situations and completes incident reports in accordance with agency policy., • In the Program Director’s absence or with the Program Director’s authority regularly directs meetings concerning clinical issues., • In the Program Director’s absence or with the Program Director’s authority coordinates the treatment team, which will include the client, family/support network, clinical staff, community mental health staff (community clinic and/or day treatment program), educational and vocational staff, and other collaterals as appropriate., • Coordinates treatment services between program and the day treatment or clinic treatment program and other community resources utilized by clients (i.e., schools, medical and dental care, vocational, recreational, and substance abuse programs)., • Maintains frequent contact with the client’s collaterals., • Coordinates and arranges transportation for home visits and client appointments, etc., • Coordinates discharge planning activities. Since 1996, we’ve worked alongside communities, courts, and those most directly affected by the justice system to build stronger, healthier, and more equitable neighborhoods. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY The Staten Island Justice Center (SIJC)offers an array of programming for both youth and adults in Staten Island, focusing on engaging those with current or past justice involvement or at-risk of justice-involvement. SIJC operates diversion, alternative to detention and incarceration, supervised release, reentry, mental health and wellness, workforce development, peer-led restorative justice and leadership development programs.
