The case manager carries a caseload of families, individuals, or a combination of both, and will deliver services to support the goals of housing stability, family stabilization, independent living, and financial security. Duties/Responsibilities: • Conduct initial intakes and assessments and annual assessments as required., • Develop initial Individual Service Plan (ISP) including relapse prevention plans with all caseload tenants. The program addresses juvenile behavioral problems, such as truancy, chronic runaways, family disputes, and curfew violations. Reporting to the Director of Practice, Programming and Partnerships, the Clinical Manager will oversee SRP Clinicians who provide intake, assessment, evidence-based group and individual programming to address participant needs and court mandates, and crisis support to participants in need. This position leverages clinical, supervisory, and program management experience to support the SRP Clinicians in the execution of their responsibilities. Job Description Position Description Responsible for the provision of supportive counseling with families residing in the New Broadway Family Shelter, the Client Care Coordinator (CCC) will conduct comprehensive bio-psychosocial assessments and other screenings of family members to inform the family’s Independent Living Plan.
