SUMMARY: Family Service League is seeking a full-time Case Manager for the Children’s Health Homes program. The Case Manager will offer care coordination to children and young adults with mental illness and/or chronic medical conditions. The Case Manager will be responsible for conducting meetings with clients to develop strength assessments and individualized planning, as well as support them with their goal achievements. Travel is required.
