Job DescriptionDescription: Job Title: Case Management Lead Department: Clinical Reports to: Clinical Director Job Summary: A Case Management Lead is in charge of leading the Case Managers in our mental health and substance use health care facilities. Duties include being responsible for ensuring clients receive optimal care and support; managing case and client files; establishing guidelines and objectives; coordinating various services according to the clients’ needs as well as develop case plans according to HRG protocols. In some instances, prepare and process documents, answer calls and correspondence, liaise with external parties, and resolve issues should any arise. Education and Experience: • Bachelor’s degree in human services or related field, • 3+ years of experience working with people diagnosed with behavioral health needs (trauma, mental illness and/or substance use), • 3+ years of experience providing services in the mental health or substance abuse community, • Familiarity with case management software, KIPU preferred, • Proficiency working on a multidisciplinary team, • Tenacity and ability to think outside of the box and work creatively to engage participants, • Familiarity with the criminal justice system and community resources, • Highly proficient in Microsoft Office applications, • Excellent verbal and written communication skills, • Superb organizational and time management, • Demonstrated problem-solving abilities, • Provide direct supervision to Case Managers, • Support case managers with maintaining case notes, case plans and all other required documentation. Conducting weekly audits to ensure quality assurance., • Responds to client related emergencies., • Works with Clinical Director and Director of Operations to maintain a person-centered, trauma-informed team culture., • Stay current with and provide detailed trainings on new program standards and policies., • Facilitates team meetings as well as attends case conferencing and all-staff meetings., • Participates in on-going training and professional development opportunities., • Anticipate program needs and take initiatives to solve program obstacles., • Supports the organization’s mission and values., • Assist with the supervision of clients in their daily activities., • Conduct Case Management Initial Assessment, • Meet with clients to complete daily progress notes, • Accountable for daily interdepartmental communication with the treatment team., • Responsible for daily chart auditing of assignment notes., • Facilitation and documentation of activity group., • Works with the primary therapists to ensure that Discharge planning and Instructions are completed throughout the client’s treatment and are clear and complete., • Assists and completes phone calls with clients and any communication with outside providers., • Conducts assessments of the client's legal, vocational/educational, and basic life needs and facilitates a bridge between the client and community resources to resolve the client's needs or issues., • Provides assistance with completing Coordination of Benefits documents., • Facilitating, recording and completing Family Pre-assessments., • Manages and documents discharges, transfers and AMAs (Leaving Against Medical Advice)., • Ensure the clients follow the clinical schedule., • Facilitating client tours of the treatment facilities., • Redirecting and reporting clients if rules are broken, • Seeks constructive feedback and has the ability to evaluate suggestions objectively., • Maintains acceptable overall attendance., • Promotes a favorable/positive work atmosphere., • Attends in-services and educational training as necessary and as assigned., • Seeks out learning experiences and incorporates new knowledge in practice., • Maintains flexibility and adaptability to expected and unexpected changes in the work environment., • Communicate effectively both verbally and in writing., • Independently solve problems, follow-up, and follow through., • Perform other tasks and duties as assigned., • Completes all competency assessments as required by the facility., • Required to maintain certifications up to date including but not limited to: (CPR, Verbal De-Escalation, HIPAA, HIV/AIDS, Infection Control), • Ensure oversight of program staff’s case management duties, creation of case management plans, and intervention strategies, • Provide supervision, coaching and development support to direct reports through regularly scheduled meetings and evaluations, • Acts as a mentor and resource to the staff when issues or problems arise, • Assist with the identification of process improvement initiatives within the day-to-day workflow to improve operational efficiencies, • Support the implementation and compliance of new policies and procedures as well as compliance of existing policies and procedures, • Identify performance deficiencies and work with staff members to develop a performance improvement plan to assist them in performing to their highest standard, • Ability to work under stressful conditions and be flexible in relation to departmental needs, • Excellent organizational and time management skills, • Proven ability to function independently as well as part of an interdependent team, • Knowledge of Joint Commission standards within the Department, • Comprehension of State and Federal Statutes Regarding Client/Patient Confidentiality Laws, • Understanding of Drug Free Workplace Policies, • Awareness of Workplace Violence Policies, • Experience with current technology, • Proficient in Substance Use Disorder The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
