In this impactful role, you will be instrumental in providing support to individuals within our supportive housing program. POSITION RESPONSIBILITIES • With Administrator, assists residents with the development and implementation of a long-range service plans, • Assists in securing and maintaining entitlements: SSI/SSD, PA Medicaid, SNAP, Section 8 housing, etc., • Conducts weekly home visits and one phone contact with assigned residents and additional support to resident(s) in crisis as needed and provides appropriate documentation of all contact, • Responsible for contact and documentation of such with collateral service providers at least once a month and as needed, • Assesses and provides referrals to residents regarding any unmet service needs, • Attention to providing and modeling the necessary development of ADL skills: budgeting, cooking, household maintenance, personal hygiene, use of public transportation, etc., • Assists in the transportation of residents to MH programs and events, • Responsible for assuring good relationships with neighbors and the general community, • Ensures that room/apartments are clean for new residents (may include cleaning, moving and setting up), • Responsible for reporting all housing maintenance issues through the appropriate procedure, • Responsible for the accurate and current maintenance of phone log, in addition to resident records, • Immediately notifies Supervisor of all potential crisis/emergency situations, • Documents all incidents and occurrences, • Conducts resident house meetings, as needed, • Conducts fire drills at each residence bi-monthly, • Responsible for assisting residents at the time of moving in/out, • Meets weekly for supervision, • Reviews residents rent monthly with Supervisor, • Attends and participates in staff meetings and scheduled in-service training and/or additional educational opportunities as well as appropriate agency and interagency meetings, • With Supervisor, responsible for discharge planning of residents, • Acts as task supervisor when interns are assigned to Supported Housing Program, • Responsible for Agency compliance requirements by grants through site visits, data collection, and tracking system, • Rotation of on-call duties, • High School Diploma or Equivalency with experience in human services required, • Bachelor's Degree in Human Services or related field preferred, • Experience working with individuals with severe mental illness preferred, • Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle, • Reports to: Community Services Coordinator SHIFT: Monday-Friday (8:30 am to 4:30 pm), with an on-call availability PAY RATE: $21.00 per hour LOCATION: Main Office in Islip Terrace, NY If you're passionate about making a difference and contributing to our mission, we want to hear from you. Key Responsibilities: Packaging Design • Design all product packaging layouts on provided die cuts, • Ensure brand consistency while adapting layouts and messaging per product line, • Create and maintain all product listings with accuracy and attention to detail, • Design and upload all visual assets for our products, including dimensional diagrams and infographics, • Ensure consistency of product images compared to similar items, • Perform photo clipping, retouching, color correction, and image optimization, • Maintain consistent look and quality across all product imagery, • Template and save out assets for multiple platforms, • Coordinate photography needs for packaging, web, and marketing, • Prepare photo shoot lists based on our current website listings and research of similar competitive products, • Provide creative direction to our on-site photographer.
