Job Description JOB DEFINITION: Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. EDUCATIONAL REQUIREMENTS: Licensed Clinical Social Workers, Licensed Marriage and Family Therapists, or Licensed Mental Health Counselors. EXPERIENCE REQUIRED: Three years' experience in mental health and/or chemical dependency field. DUTIES AND RESPONSIBILITIES: • Charged with supervisory and administrative responsibilities to ensure that staff meets the standards and requirements of the Agency and appropriate funding and regulatory bodies., • Direct and supervise staff activities, lead team organizational and service planning meetings and provide direction to staff regarding individual cases. Conduct side-by-side contacts with staff and regularly conduct individual supervision meetings., • Responsible for overseeing Assessment and Service Planning, Quality assurance and Improvement Processes, Utilization Review, Incident Reporting, coordination between FACT and other systems., • Engage in performing assessments, handling crisis intervention and discharge planning., • Provide direct, face-to-face and/or collateral services to recipients either in the FACT office or in the community in places and situations where problems arise., • Must be available (along with other members of the FACT Team) on an as needed basis 24 hours a day, 7 days a week.
