Job Description RiseWell Community Services is looking to hire a Full-Time Care Coordinator for the Care Coordination Health Homes, serving Long Island region and reporting to the program supervisor. Our Care Coordination teams help individuals with serious mental illness, chronic medical conditions, or substance abuse disorders. We connect participants with behavioral health specialists, primary medical care, housing services, vocational support, and entitlements. Our goal is to make sure the participant stays on top of appointments and receives the vital care needed for a healthy life. QUALIFICATIONS: High School education or equivalent or CASAC with at least 2 years of experience serving people with mental illness, developmental disabilities, alcoholism or substance abuse. JOB FUNCTION: Provide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions). What You’ll Do as a Dental Office Receptionist: • Frontline Communication: Operate the multi-line telephone system - answering, screening, and directing calls, taking messages, and scheduling appointments, • Patient Interaction: Greet visitors and callers warmly, address inquiries, and efficiently direct them to the appropriate team member., • Administrative Support: File and maintain records, sort mail, manage courier deliveries, and accurately input data into our systems., • Technology Adoption: Learn and utilize new office technologies, including digital charting and our Denticon system, to support daily operations., • Financial & Clerical Duties: Balance credit transactions and provide basic financial support., • Professional Presentation: Use designated scripts effectively both over the phone and in-person to communicate information about our practice and services.
